Soon after the transplant you need to call the CMS COB Contractor in New York, 1- and tell them about the transplant so they can send an inquiry to the patient. Once they get back the info about the transplant they will put an ESRD flag on her account and Medicare will drop to secondary. This will cut down on the time lag of waiting for the the patient to send in the 2728 form. Fred L. She has just started hemodialysis, so will not be eligible for ESRD Medicare for 3 months. It is likely that she will be transplanted before 3 months and be eligible for ESRD Medicare beginning the 1st day of the month of the transplant. This will be the beginning of the 30 month COB. and the small group healthplan will be primary. For awhile after the transplant Medicare will not have the ESRD Medicare eligibility information. At the time of transplant Medicare will think that they should be primary, due to medical disability Medicare and a small group healthplan, correct? What should I do about pre op registration? Can I register the patient with the group health plan primary, since it will be when social security to notified of the transplant and the 30 month co-ordination period begins. This can take awhile. This all sounds so confusing, I hope it make sense to someone! I'm just trying to head off a billing nightmare.
