I will almost bet it is Intracorp that did the medical review for this

carrier...did I guess right? I have a similar situation on a case that I

am currently working on. Dr. Habeeb at Intracorp was adament that the

patient go through a full psych eval, even though our multidisciplinary

team passed her for listing.

If the insurance has denied, the patient should contact his employer

group and appeal the denial.

We have a similar patient with CIGNA too. They denied his tx as secondary payer because of compliance!. if and when Medicare becomes primary, will the insurance co. cover the

out-of-pocket expenses or

continue coverage after 3 years ?

In Oregon the payment of immunos under Medicare HMO's (or Medicare +

Choice or Medicare Advantage) follows the coverage for DME since that is

how immunos are paid for. Whatever the DME benefit is will be the

immuno benefit. This is because immunos are billed as a DME product so

fall under their reimbursement guidelines. This only became an issue in

the last year as HMO's have reduced their DME benefits in some cases to

below 80%, which they are allowed to do.

Medicare supps are different. They have to pick up the 20% after

Medicare pays 80%. We have had to recommend pts switch to supps instead

of HMO's. But this can be tricky because of the various open enrollemnt

rules.

Of course I don't know how this translates beyond state borders.

My experience has been with our Medicare HMO plans is that the immunos

do not fall under the prescription limitation. They have to follow the

Medicare rules, and since Medicare covers the immunos, they will as

well. Actually my understanding is they go through Medicare, not the

HMO. I find this alleviates most of their expense, especially once they

get settled with only being on a couple of meds.

I am starting to see this more and more, and would like to know who

your

centers handle these cases.

A patient has a managed care medicare, but the drug coverage is very

limited

eg. 50% co-pay and 2500.00 per year coverage.

You explain the cost of the meds, and know they really cannot afford

the cost

of the meds when they meet the max., and

they still insist they want to be listed, and most pharmaceutical

companies

will not help, since they have some type of drug

coverage and they earn too much to qualify for help and too little to

pay the

full cost of these meds. Having them sign

anything stating they realize the costs and accept responsibility still

will

not get them med coverage. Any suggestions?

What we do with medications is, list what is needed post transplant

and what could potentially be needed (ie, toprol, lasix, fosamax,

plendil . . . ) We display on our copy the price that Eckerds or

Walmart Pharmacy has quoted us for a thirty day supply. We show what

medicare would pay for immunos and the difference. We give the pt the

med list without the price and tell them to go to their pharmacy and

using whatever their drug plan is, find out how much the medications

will cost them the first month after transplant.

touch on the medical part but also on the medication side post

We do almost the same thing. They are given a list of meds they may

have to take (including immunos) which includes the price for 30 day

supply along w/their estimated copays (obtained from ins.co). We

started doing this approximately 6 months ago as patients do not realize

how much their copays are until they see it in black & white. You can

talk till you're blue in the face, but when they see it in print and add

up the $$ it really hits home!

we've developed a short and long term financial planning packet and included a worksheet with room for the pt's current living expenses and a separate box for new expenses related to transplant which we ask them to fill in with our help.

thank you for your wise words. I think I need to explain how I look

at this voice in my innner. I don´t know if this is God or not. My

first feeling was. Bur now I don´t really care. Someone is listening

Hi Cammie -- I'm going by memory now, but I think there are

two tests that measure the same antibodies -- anti-thyroid

thyroglobulin and the TPO (thyroid peroxidase). So if either is

elevated, you have Hashimoto's, to my understanding.

Finding an doctor who treats by symptoms and not labs is always

a challenge but see the description I put in the main page of this

Also, Shomon's site has a section for patient referrals to

I have a GP (not an endo) currently who has let me try a higher

dosage and disregard the TSH, but he does so hesitatingly (docs

are afraid of getting sued and treating by these labs has been the

accepted practice since 1975). I'm frankly tired of trying to figure

out what's going on on my own, so I'm looking for a new doc too,

someone progressive. What's the dosage your doc has you on and

how long have you been on it? It can take a while to kick in but if

you don't start seeing some improvement after a few weeks of

treatment, I would either decide if you want to arm yourself with

the information and convince your current doc for a boost, or

find someone new. Good luck!!!

Hi from another (Australia = Oz)

Just thought I'd comment.... after many years of telling doctors my

thyroid had a problem and them telling me it was in my mind even with

blood tests, I finally got a doctor who looked at previous tests and

said they had all been idiots to not see it was Hashimotos. Took me

over 8 years to get treatment.

But about the blood test..... I've actually been taking twice the dose

my doctor put me on because I know and feel its right for me. My blood

test showed it in normal levels. So occasionally you really do have to

listen to your body, not the doctors.
