All circs where I work are done with blocks...We also use a sucrose solution, given orally to babes. (Believe it or not, thanks to one of our peds who heard about this at a ocnference about 10 years ago) There is literature that shows that sucrose solution given po to a baby at least 10-20 minutes prior to the procedure, works on the opiate receptors in the newborn brain....we also allow baby to suckle on these homemade sucrose suckers for awhile after the procedure. We rarely have babies that cry during procedure. We only restrain the baby's thighs, hands are allowed free, usually the baby keeps them well out of the way, up by the face. And parents are allowed!! Procedure is done on a padded restraint under a radiant warmer. Baby is immediately taken to the mother for breastfeeding or skin to skin for comfort. Circ checks are done (with baby on the Mom or Dad usually) for q 15 mins X 1 hr. The docs use either traditional Gomco (for which a mixture of lidocaine jelly and vaseline is placed on the dressing for 24 hrs) or plastibell (no dressing). Tylenol is ordered prn and we give it when we think baby is unsually fussy...which is pretty rare. Most babies are quiet, sleepy for the day of circ. This corresponding with less feedings or shorter feedings for that day. We prefer not to circ any baby that is sleepy and/or having breastfeeding difficulties. The permit for circ is usually not signed by the parent until feedings are established (this keeps the " well meaning " ped from hurrying the circ along! We try not to circ them until the AM of the day they are going home) We do not use EMLA cream for circs, it is clearly stated in the accompanying package literature that it should not be used on the penis. Our RM dept gets alittle squirrely about us using things in a way that have specific written contraindications. We do use EMLA cream for any IV starts on newborns (and ped patients), it is important to remember that for the EMLA cream to be effective it seems to require at least an hour of time in place before the start therefore not good for those stat starts. I used to work for a group of Peds where one of them, a very devote Jewish Dr, would give the babies a nipple full of Manesheviz wine just prior to the circ.
