I would love to start a new mailing list for OB/Peds home care nurses. I am a manager of a new home care program and would love to learn from your wisdom.

It's alot cooler now

boy am i jealous..the Florida heat and humidity is a killer. A cool day for us is any day that's overcast and 93!!!

We do peer chart reviews here in all 3 areas (PP also does gyne so they review a pp and a gyne chart) We have to do 2 a month, and if we do them all we get a point towards our Professional Practice Program (PPP/career ladder)Somehow they tend to "lose" quite a few of them lately.

Our PPP gets us 50 cents an hour for 5-9 points, $1.00/hr for 10-14 pts and $2.00 for 15+ points. We get a coaching plan every 6 months that evaluates our points. If your points drop you down to the next level, you lose $$$. Laurie

It's alot cooler now

I only do home care prn, with Matria, and that is mostly

high risk antepartum patients but would love to get more

information from others doing home visits, count me in

ohio

" merge " with Healthdyne. I had applied to the local office, but while I

would have enjoyed it, their requirements for weekends and holidays even for

per diem staff made it not worth me taking a position. Too bad--I really

wanted to do it.

would have enjoyed it, their requirements for weekends and holidays even for

per diem staff made it not worth me taking a position. Too bad--I really

Also, in regards to task sharing..I work in a combined

maternity unit, LDR, NBN, and PP, so have a hard time

envisioning a larger staff than ours!! One of the

things I would focus on, however, if I had the

opportunity to actually delegate would be

my unit is combined LDR, PP Everyone (except usually LPN's) rotate between LDR and PP. I usually spend 4 of 6 days as LDR nurse. Everyone has assigned tasks acording to postiont the #1 LDR nurse is in charge and is the only nurse there if there is just 1 patient or 2 observation with no problems she's responsible for checking crash cart on days and everyshift #1 takes care of census and report to supervisor. #1 PP is in charge there and depending on shift makes sure certain CP's are updated. #2 PP is med nurse, #2 LDR takes care of patients, #3 LDR is for scheduled sections. Then there are cleaning assignments, stocking, putting up scrubs.

When I started here I wasn't too crazy about the way we do PP asignments...we don't have an assignment. Everyone just kind of goes together for VS and when it's time for assessments everyone just starts at a point and we do them till they are done then regroup several times during the day and give a little mini report since I've gotten used to it it's not too bad.

PPP/career ladder

there is hardly anyone the does ladder at my whole hospital they make it a pain in the butt to do. They have recently changed the way it's done but it's LOTS of extra work you have to turn in samples of your charting to the committee do inservices that aren't mandatory but have so many attend, attend inservices, hardly any points for more education or certifications, be on committees which may already be full just a general pain in the butt. What do you have to do for ladder at you hospitals?

How many patients do you have on postpartum? This sounds kind of confusing.Who is accountable for the patient? We have a 50/50 postpartum gyne mix, and during the day we have outpt gyne surgeries on our unit (procedures are done in the main or) so that staffing probably would not work for us. Do you do couplet care? Just curious =) Laurie

we signed up for call here. You would end up with a weekend call about once every 3 months and a weekday call twice a month with every 3rd month either no call or 1 day depending on staffing at the time.

How many patients do you have on postpartum

PP/GYN has 16 beds. If full of PP gyn has to go to general surgical unit. The person assigned P 1 (postpartum) is responsible to make sure all assessments are done, make rounds with docs (others help too) and give reports and receive new surgeries. We do our own c-sections and post partum tubals. LAVH etc goes to outpt surgery first then we get them post op. Day evening and nights has 2 staff maybe more if LD isn't busy then they come to PP days has more depending on LD but always 2 unless LD is really busy then maybe only 1. Everyonce in a while everyone ends up in LD with someone running back and forth answering the PP light but only if there are just a couple of patients otherwise we call in more help. We have a call nurse from 3p-7a and all day on weekends incase labor gets busy and there is nobody from pp to call for help

This was about 2 years ago, and at that time from what I remember, the

requirement was 1 weekend a month and 1 day a week and 2 winter and 2 summer

holidays.

I have a full time job, so any secondary one I take really can't take up the

rest of my freee time.

I think I acquired HCV while in the hospital last December, but that's a

whole other story. The " official " version is that 6 months after my

hospital stay, my doctor " suddenly discovered an old test " taken in

December that revealed I had HCV. (By then, I had been on Lipitor for

months, not the best situation.) I never once felt any of the symptoms

normally associated with HCV, not then, not now.

I don't know about this one either. My biopsy really HURT and they didn't

want to give me much for it, which is probably the best thing for my

liver. But once I got home, the pain was gone--poof--all I felt was relief

that something I dreaded was over with. In fact, once they let me sit up,

I was pain-free.

I wonder if your current feelings might have more to do with concern for

your health and your future? I know that for probably a couple of weeks

after my diagnosis, I kind of sat on the couch in a state of dazed

depression. I'm not sure for how long, but I don't think I got out of my

sweats and into street clothes for awhile. I know I personally had

thoughts of, " it's all over, I'm done, stick a fork in me, woe is me " etc.,

etc. If you allow it to, it will pass, and you can begin taking some

positive steps that might or might not include taking the treatment, or

seeking alternative treatments--whatever you do, you should know that a lot

of people are here to support you. These are terrific folks here on this list.

(By the way, I'm not saying it's wrong to be depressed over what's

happened--in a way, we're grieving for a way of life that is probably

changed forever. Even so, it's probably best not to let that process drag

on over-long. And it's better to know, than not to know. Think of it this

way: life from here on can be what we make of it, so there's no point in

wasting time on regrets. )
