First things first.

1) You will need a letter from the Urologist who is taking care of the donor that certifies that the complication is directly related to the donation of the kidney.

2) Next on the 1500 form you put in Block 2 the donor's name, bock 3 date of birth etc.

3) Block 1a - Medicaere recipient's ID number, block 4, the recipeitns name, bock 7 add, block 11 Medicare and under that the recipeint's date of birth (Insured's date of birth).

Then in Block 21 the first diagnosis code is v42.0, then V59.4, then with every procedure code a Q3 modifer must be with each procedure code.

P.S. - a, you have to send this on a paper claim and request it be hand processed as the Medicare computer can not process this claim as it will have fatal errors, i.e. patient name does not match Medicare Number. Bev>>> a.Summa@... 11/9/2004 3:28:13 PM >>>

Also, hopefully this is JUST for the

physician services. Any facility charges are not billed to anyone, but

are handled just as if the donor were being evaluated, that is written off to a

contractual allowance and logged for inclusion on the Medicare Cost Report

Worksheet D-6 Part I for kidneys.

You are absolutely right. We had a donor with nerve issues post donation and this took a great deal of time to find out the ins and outs of trying to get her claims paid. This was all done outpatient. Bev>>> Vaughan@... 11/9/2004 7:02:38 PM >>>
