If you are paying your dues & registration @ the same time, you MUST send BOTH pages of the registration form AND an '05 Member Enrollment form WITH YOUR PAYMENT. We've had forms arrive w/o checks & checks w/o forms, so unless you even more e-mails from me... :-) I DO know the problem is likely w/the AP dept since mine has a history of doing the same thing, but please remind THEM they need to send all required forms w/payment, ok? 2. Checks should be made payable to TFCA but should be sent to UNOS Travel (we've had checks, w/o forms, show up made payable to UNOS, which is causing bookkeeping problems on our end). 3. Please carefully review the tuition fee schedule before submitting a check request. The 10% discount for a 2nd or 3rd member from the same ctr is ONLY applicable to the Workshop registration fee-ALL members MUST pay their individual dues IN FULL; no discount is applicable for Membership or Associate Membership Annual Dues. Also, the 2nd & 3rd members must EACH complete the 2-page registration form AND an '05 Membership Enrollment form, even if you are renewing your membership (phone#'s change, e-mail address' change, departments are relocated, hospitals institute new internal mail policies, etc).
