The 'slip' was noticed because I had been extremely lax and hadn't been back for a review for a couple of years (living 4 hours away, small children, lots of other stresses to look after to I put them all before me). And even worse I knew my band was way too tight. As part of my getting back on track my band doctor ordered a barium swallow and that how it was found.
