I got much better results and take 4.5mgs by dissolving the Revia 50mgs pill and measuring out 4.5mgs. If you take compounded, open the capsule and put in water or other liquid and drink. Also, I found the generic naltrexone didn't work for me. I use the Bristol Myers Revia. There is another member that tried 25mgs and says made her stable. She doesn't take that amount regularly tho and takes the 25mgs in day. She may reply to you. Or could be the next phase of healing more stubborn items. Ups and downs... but in tracking my own progress I have found that the worst of days with LDN were still 100% better than my best days in the 3 years prior to starting it. It's not perfect, but nothing me and my chiros haven't been able to work with. If you stay with it, I hope you start seeing positive changes again. Good luck. You need to go to 4.5mg. With you being PPMS you really need the 4.5mg dose and MAY possibly need as much as 6.0mg if 4.5mg is not enough to halt your progression. Make sure your pharmacy is using pure naltrexone powder in your capsules and not grinding up ReVia pills. Also make sure your pharmacy is using one of these three fillers in your capsules...lactose, acidophilus or avicel. I am a patient of Dr. Bihari and I get my LDN from Irmat. It was Dr. Bihari who told me to use calcium carbonate and wrote the prescription. I will talk to Irmat about the issue of packing the filler too tight. As of yet I have had no problems, in fact the dry heaving Steve refers to stopped when I switched to calcium carbonate.
