I don't know how it is other than So. California but we turn the pt to the

side (start with left then right if no better) instead of trendelenberg. (the

rest is the same).

We run 20 u. Pit in the IV after the delivery of the placenta and if the mom

starts to bleed heavily we give of 0.2mg Methergine IM.

I think probably you'll find most places practice slightly different.....we

pushed pitocin IV several years ago....like in the 70's but not now think

they decided it could cause hypotension, cardiac arrhythmias....our usual

first line with hemorrhage is Methergine IM...then Hemobate. We also do not

place in trendelenberg unless we suspect a cord. Just know that most places

do practice slightly different.

I work in Northern CA and our unit is the same as s:

turn to left OR right side, no trendelenburg.

Hi,

I am an OB nurse. In our hospital we rotate between labor, post partum, and

nursery. I love L & D. I am thinking of looking into becoming a midwife. I

have my BSN and I am located in South Central NY. Anyone have any info on

classes available not too far from me? 2 hours drive is not bad to me. So

that would include Syracuse and Rochester area. Thanks for any help anyone

can give me.

(actually, one just passed her boards and her comps this summer). I'll get

Thankyou all for responding. just got back from a short vacation...but

wanted to respond to some of the notes i got....actually i did mean ivp into

a main line of 1000cc lr with another 20mu of pit in it...i'm guessing this

isn't standard practice anywhere else?

I thought it was contraindicated to give pit direct IV push.

I have always understood that it needed to mixed in IVF and administered.

It can be given as a bolus then.

It is not the practice at my hospital to give it without mixing in IVF.

I'm glad you don't do this at your new hospital.

As far as trendelenburg, I think LLP or RLP works very well. We don't do

many epidurals, but, I would never put a patient that had an epidural in

trendelenburg because it would cause the meds to drift higher causing a high

spinal type effect, and risk respiratory depression or failure. I have put

pts without epidurals in trendelenburg if having severe hypotension,

possibly due to hemorrhage postpartum. Epidurals tend to cause decels in

the fetus due to hypotension in mom, usually it is just transient. At least

you hope so when it occurs :)
