And May I add YUCK!? 99404 PREVENT MED CNSL-SEP PROC; 60 MIN Is what I have been using so far and have gotten re-imbursed. > V25.09 GEN OTH CNSL & ADV CNTRACPT MGMT is what I have changed to because I found that V26.4 GN CNSL & ADV PRCREATIVE MGMT was being treated as an infertility code. If your client has physical signs and symptoms, like headaches, etc, you could code for those things as well. I am not sure what the FC at the end of your numbers means? I could not find the H1010 code either. In addition to you calling the insurance company, I recommend you have your *client* call them as well and tell her to complain royally! Ask the company what range of codes they reimburse for family planning. They may not tell you a " specific " code because then they could concievably get in trouble, but if they give you a range, you can pick through and choose the most appropriate code. H1010 should be listed for the CPT code. And no modifier is used for BCBS.
