Dynamic Listening Systems.

Summer 2005 Outreach Program

Thanks for your interest in our programs. This sheet

outlines the dates, structure and fees for the

upcoming summer outreach programs in the South Bay and

San Valley areas. The exact locations are

being determined, and will be announced shortly.

The program will consist of an assessment plus 30

sessions.

The schedules includes a three week break between

blocks as follows:

Simi Valley & #150; 1st block of 15 sessions & #150;

June 20 & #150; July 8 (Monday through Friday)

2nd block of 15 sessions & #150; August 1 & #150; 19

(Monday through Friday)

South Bay & #150; 1st block of 15 sessions & #150;

July 11 & #150; 29 (Monday through Friday)

2nd block of 15 sessions & #150; August 22 & #150;

September 9 (Monday through Friday)

Session will be:

9:00 & #150; 10:20 am

10:30 & #150; 11:50 am

12:30 & #150; 1:50 pm

2:00 & #150; 3:20 pm

4:00 & #150; 5:20 pm

5:30 & #150; 6:50pm

You will pick a session and come every day Monday

& #150; Friday for 3 weeks at the time you are

scheduled for. That will make 15 sessions.

The assessment should take place in our Beverly Hills

office prior to program start if at all possible. We

will mail a detailed developmental history to the

parents and ask that it be completed and returned

before you come in for the assessment to help us

understand more about your child. The assessment will

include a Listening Test and other

evaluation/assessment tools to help us understand

important functions like sequencing, motor planning,

coordination, sound sensitivities and auditory

processing.

Each listening session will include the delivery of

specially filtered and modulated music via headphones

that include a bone conduction component, plus

structured play, O.T. activities, to encourage

vestibular stimulation, sensory integration, auditory

processing, sound sensitivities etc. There may be up

to four children in any session, and they will be

encouraged to interact to the best of their ability.

The breakdown of the fees is thus:

Intake Listening and O.T. evaluation and consultation

$ 300.00

1st blockof 15 sessions --- 1hour 20 mins

2nd block of 15 sessions --- 1hour 20 mins

per session M,T,W,Th,F $ 1,125.00

Total for 1 program of 30 __________

sessions $2,625.00

Payments will be due as follows::

$ 300 will be due on the day of the assessment.

$1,125 will be due during the first block of 15

sessions.

$1,125 will be due during the 2nd block of 15

sessions.

Payment may be made by cash, check, Mastercard or

Visa. A superbill for submission to your health

insurance carrier will be generated at the end of the

program. You may be eligible for reimbursement,

depending on the terms of your policy.
