Sorry, I've been stockpiling. 12yo male initially presents with pinworms x 2years. Family had tried all kinds of OTC treatments, but he had constant sensation of crawling in his anus and would frequently see worms. He also had periodic abdominal pain - RUQ - not able to associate with anything in particular. He was 4'9 " and 79 lb; small for his age. His energy is at times lower than his mom thinks it should be and he has very little appetite. Parents force feed sometimes. He is very picky and eats very small portions. Only food he craves is steak. He appeared pale in the first visit, almost yellow. He gets grumpy when he's tired, will yell, nothing will please him. Generally bright, communicative, funny. When 11 months old, developed an infection in his left hand, was put on antibiotics, worsened and ended up in quarantine in hospital as they couldn't get an accurate culture. Ever since, his middle Left finger doesn't bend at the phalangeal joints. His baby teeth were ruined from the antibiotics and had to have them all removed or capped. At 9 yo he developed shingles. He had had chicken pox as a younger child. Labs: Sodium/Potassium ratio - 26. Serum chloride 103. Calcium 10.2 Alk Phos 238 AST and ALT - WNR Ferritin - 32 TSH - 4.157 Free T3 - 3.9 (range for this age is 3.1-5.9) Free T4 - 1.22 Vitamin D 10.8 CBC Normal 2nd visit - pinworms resolved. No sensations and no sign of worms for a couple weeks. Still having periodic RUQ pain - shooting. Abdominal exam benign. No change in stools. BM's are always stinky as is gas. Appetite improved some, but not dramatically. Sleep great, though gets to bed late a few nights a week. He is having a hard time focusing at school according to his teachers, but often doesn't eat breakfast. Treatments: COP's, Unda 3,17,50, Bioenzymes between meals and pineapple juice tx for pinworms. Vitamin D - 6000IU/day 2nd Visit: COP, Probiotics, Liquid B, Breakfast, Unda 16, 34, 1000 and sleep hygiene. My question: Are any of you familiar with adrenal and thyroid stuff in kids? Any suggestions? Mom has been reticent to seek any medical care in the past, but seems very on board now. Pt is also compliant and fun to work with. I am just concerned that I am missing some greater underlying pathology contributing to his low appetite, fatigue and small stature. I appreciate this forum and your help. Thank you.
