Evaluation Criteria for Behaviorist

Hello! I was wondering if anyone has come across a sample survey of

questions for a family to ask possible behaviorists? One of my

student's parents have asked for names of behaviorists but my

district has asked that I not " recommend " anyone. Instead, they

asked that I give a list of behaviorists as well as agencies and

recommend some questions they can ask to decipher who to hire. I

have already referred them to the listings of certified behavior

analists and have developed somewhat of a list of questions. Just

seeking anything more OFFICIAL which lacks my personal opinion.
