Here, our docs are actually talking folks out of circing their boys. Which

I am damn glad about. I don't think it's a time issue, I think they are

realizing it's truly not medically necessary.

At any rate, I think our hospital now does circing at about 40% rate, which

is pretty low, isn't it?

In Louisiana where I am, we do about 95% of circs. Mostly without anything

(block). Some of the docs, mostly the OBs, use Emmla cream one hour prior to

the circ. We still have many OBs that do there own circs and our Peds Drs.

are OK with it. They hate to do them. I don't think it is because it is

controversial, I think they just plain hate to take the time when they have 4

other hospitals to make rounds at before their own practices open!

I'm glad to hear that as 10 years ago, when I taught a class in Newborn Care,

I was telling the parents that they had a choice. I didn't recommend one

thing or the other, but explained that is this day and age, circs weren't

necessary.

Boy did I get yelled at by the Docs, as it was said that I was infringing on

their territory. I decided to quit teaching the class and just go back to

regular labor and deliver. I'm glad that things are at least starting to

change.

Hi, Here in the UK we don't circumcise at all unless the parents can produce

good evidence for it.

I think this is barbaric! All docs at our facility are required to use not

only a block but also a sucrose pacifier. Is anyone else using the sucrose

pacifier yet? We have been using it for about 5 years. Most babies are

content and rarely cry through our circs. And for the " granola " town I live

in, can you believe we have a >95% circ rate? I can never get over that. The

Dad's or Mom's accompany the baby to the procedure room for the circ. The

only babies that don't get circ'd seem to be medical professionals kids.

Personally, I would like to see us get back to nature on this one! I don't

think it is a necessary procedure and I equate it with female circumcision in

Africa.

And that's my opinion and I'm sticking to it! Heehee,

We also have prn tylenol orders for post circ, although not used very often.

We do not restrain their arms, just the legs, they like having their hands up

by their faces and never reach down so it works fine. I wish I could refuse

to assist with circs, I hate them. I have been lucky enough to avoid

assisting for years as most are done early in the AM, by 6a or 7a and I'm

usually doing something else and I send the Unit Assistant to help. I cheat,

I admit it!

Jan..do you make your own sucrose pacifiers or purchase them? We made our own

for one physician..3 sterile cotton balls in the nipple with about 5 cc

Glucose water on it and plugged with a servo probe cover...I really didn't

notice any difference in behavior during circ. Lori

Betsy..about 6 yrs ago Florida Medicaid didn't pay for circ's either, and the

ped's wanted $100 cash up front..our peds actually wanted the parents to bring

cash or a money order to the nursery for the nurses to hold until the circ. Me

and my big mouth protested..I am not the cashier's office and if anything

happened to that $$$ you know who would be blamed. They wrote a policy where

parents had to bring $$ to the office of the ped or have bay circ'd in office

as outpt..we were relatively circ free for about a year (except the private

pts who's ins. paid) Then medicaid picked up again and paid. Since Medicaid

paid only $35..the peds wanted to collect the other $65..again we refused, now

they grumble, and they let the FP residents do them....ouchie!!! Lori

OK here I go with my opinion. First can you really compare these two

procedures? I thought a female circ was to prevent the woman from enjoying

sex so she would be a " good " and faithful wife.

I realize that both procedures are cultural and I think that the opinions of

parents should be respected. Even when we don't agree with them.

Second, I was one of those women who chose to circ my son. And yes, I had

seen it done, had held the kids down myself.

Next, what is so bad about an epidural/intrathecal? Why do we think that

women should just accept the pain of childbirth? Some women just are not, nor

ever could be prepared for the pain! I was luck and had very short labors

with my children and did well with natural childbirth. But I don't hesitate

to offer pain relief to my laboring patients.

Don't get me wrong I support ALL the comfort measures and I do agree that

walking and tubbing and rubbing can do wonders but I like epidurals and

intrathecals for my patients who want them

OK done venting now.

I, for one, agree with you.

After working labor and deliver for 22 years, I've seen it all. We have

several docs that let the patients plan their labor and delivery and even let

the Dad's help. (They let them cut the cord and be down at the end of the

table with them.) I think the wives would rather have them up with them

holding their shoulders and wiping their brow, but whatever.

Anyway, even after the gals have gone through, sometimes up to 20 hours of

hard labor, if the fetal monitor looks all right or sometimes we just put the

monitor on for 5 minutes an hour, they still ended up going for a C-Section.

These girls were so worn out that it took them a long time to recover after

their C-Sections, compared to the girls that had the epidural and experienced

slight pain and then had C-Sections, if they couldn't deliver vaginally. Also

we had a lot of V-Backs, but we didn't pit them and they were only allowed, if

they weren't CPD, but just breech, PP, etc.

Thanks, ! Believe me, I have thought about it! But then I figured the

heat would wreak havoc with my asthma!

Of course, New Jersey is NOT the best state for living in when you have

asthma..........
