I don't think it's a time issue, I think they are realizing it's truly not medically necessary. At any rate, I think our hospital now does circing at about 40% rate, which is pretty low, isn't it? Some of the docs, mostly the OBs, use Emmla cream one hour prior to the circ. We still have many OBs that do there own circs and our Peds Drs. are OK with it. They hate to do them. I don't think it is because it is controversial, I think they just plain hate to take the time when they have 4 other hospitals to make rounds at before their own practices open! Guest guest Posted March 16, 1999 Report Share Posted March 16, 1999 I'm glad to hear that as 10 years ago, when I taught a class in Newborn Care, I was telling the parents that they had a choice. I didn't recommend one thing or the other, but explained that is this day and age, circs weren't necessary. Boy did I get yelled at by the Docs, as it was said that I was infringing on their territory. I decided to quit teaching the class and just go back to regular labor and deliver. We made our own for one physician..3 sterile cotton balls in the nipple with about 5 cc Glucose water on it and plugged with a servo probe cover...I really didn't notice any difference in behavior during circ. Guest guest Posted March 19, 1999 Report Share Posted March 19, 1999 Betsy..about 6 yrs ago Florida Medicaid didn't pay for circ's either, and the ped's wanted $100 cash up front..our peds actually wanted the parents to bring cash or a money order to the nursery for the nurses to hold until the circ. Me and my big mouth protested..I am not the cashier's office and if anything happened to that $$$ you know who would be blamed. They wrote a policy where parents had to bring $$ to the office of the ped or have bay circ'd in office as outpt..we were relatively circ free for about a year (except the private pts who's ins. paid) Then medicaid picked up again and paid. Since Medicaid paid only $35..the peds wanted to collect the other $65..again we refused, now they grumble, and they let the FP residents do them....ouchie!!! Don't get me wrong I support ALL the comfort measures and I do agree that walking and tubbing and rubbing can do wonders but I like epidurals and intrathecals for my patients who want them OK done venting now. Guest guest Posted March 19, 1999 Report Share Posted March 19, 1999 I, for one, agree with you. After working labor and deliver for 22 years, I've seen it all. We have several docs that let the patients plan their labor and delivery and even let the Dad's help. (They let them cut the cord and be down at the end of the table with them.) I think the wives would rather have them up with them holding their shoulders and wiping their brow, but whatever. Anyway, even after the gals have gone through, sometimes up to 20 hours of hard labor, if the fetal monitor looks all right or sometimes we just put the monitor on for 5 minutes an hour, they still ended up going for a C-Section. These girls were so worn out that it took them a long time to recover after their C-Sections, compared to the girls that had the epidural and experienced slight pain and then had C-Sections, if they couldn't deliver vaginally. Also we had a lot of V-Backs, but we didn't pit them and they were only allowed, if they weren't CPD, but just breech, PP, etc. Quote Link to comment Share on other sites More sharing options...
