The ndc #s on the claim form are: 1: methylcobalamin powder ndc#51927-2963-00 2: water for injection (sterile) inj ndc#0074-4887-10 3: sodium chloride 0.9% inj ndc#08021-5135-74 4: syringe, insulin bd ultra-fine II 0.3-mil each ndc#08290-3284-38 This may be more than you need, but I hope it helps. They sent an extra shot for me and it really didn't hurt at all because of the ultra fine needle.
