Yes, immediately download the 'Request for Reconsideration' form off the SSA website, Form # SSA-561-U2 fill it out, make sure you word your statement on why you disagree with the first decision carefully - stick to why you are 'unable to work' and send it Certified Mail/Return Receipt Requested. Do NOT miss your 60 days deadline! Go to your doctor's office. Ask for your file. If you have to stay there all day, use their xerox machine and make copies of every scrap of paper in there. You need this information, also SSDI needs this information, plus extra information that you have. Make sure you've got copies of all your tests, EMG/NCV, blood work, whatever else you've had, doctor's reports/letters. They may make you sign a release to get your own files, just sign in. If your doctor's office won't assist you in this, you may need legal help now. I filled out the reconsideration form and the med. release forms (they mailed to me) last night and sent them off this morning. I put my reason for disagreeing with the decision was because I thought that they didn't look at sufficiant medical records to determine the decision. I hope that was alright. I will see what happen with this. I hope it doesn't get denied again but if it does then I will contact a lawer and go that route. On a more personal note, I really appreciate all that you do.
