The seminar has been approved for CE credit (1.5 contact hours or 0.15

CEU). If you would like to have the CE credit, send a check for $6.00

payable to

NC OSHERC, a completed evaluation form, and

your PID. If you do not have a PID, I need your full name, date of

birth, and email to create one. The deadline to submit the materials

for CE credit

is February 19, 2010. Mail all items to

Randolph

using the address below.

The seminar has also been approved for 0.25 CM points by the American

Board of

Industrial Hygiene. If you would like CM points, you need to submit a

completed evaluation form; there is no fee. I will need your mailing

address to send the CM certificate.
