The first visit is free and the lawyer can explain your various options. Which doctor knows you the best and listens to you? If it is your primary doctor then have him fill out the form. He should have reports from all the doctors you see and that should help him complete the form. Make and appointment and have the doctor fill it out during your visit. Do not tell them that is why you are scheduling the appointment because they will just say to drop off the form. Just make an appointment for a routine check up. Don't show the form to anyone before your doctor comes in the room because once again they will just take the form and fill it out later. Just tell the doctor that you wanted to be sure the form is filled out with your input as well as his. He may balk a wee bit but you have a talent for humor so turn on your humor button and plenty of charm.
