Are you talking about immunos? I had a

pharmacy yesterday that had to run it a few times before copay would come back

$0 for immunos – something was wrong with the computer program. It

finally went through.

I am dealing with that today. The Medicare B wet through and Medicaid kicked it out. The pharmacy was going to call Mass Health and see what the problem is. It may just be Mass Health Kelbe.

I tried to find you at the conference last week and could not. Sorry I missed you.

Yes, this is for Prograf, Medicare part B

paid a portion, part D denied it and when they tried to run it through

Massachusetts Medicaid (Mass Health) as a last resort, we were told that Medicaid

does not pay Medicare copays. This is news to me.

Medicaid should pay the 20% -- it did

finally go through yesterday. A patient called me in a panic with an over $100

copay.

It was my understanding that the medicaid policy has to be straight Medicaid

without a carrier if Medicare is the primary insurer.

Not unless the Facility, Transplant Surgeons (physician org.) including

medical services,etc. are participant. In other words an active contract

between the

managed care plan and medical services mentioned previously.

What our "approval" from the Medical Director of the Fiscal Intermediary really looked like in writing is this (abbreviations are mine):

"...Medicare may neither pre-approve nor otherwise preauth any service or treatment. However the info you submitted regarding Ms X's medical history, and the pertinent literature as well as supportive comments from Medicare consultants indicate such treatment in this pt is medically reasonable and necessary, consistent w/Medicare requirements....

If reimbursement for this tx is denied, please submit the denied claim as a redetermination according to the process... I will approve coverage if the claim comes to me for a redetermination...

I am getting conflicting information. I just got off the phone w/ Medicare and they do not pre authorize medications. She basically told me that it would be something that the providers would need to pursue in the event it is not covered-after the fact.
