1. Mt. Sinai in New York City to provide Donor clinical/info. for HIP

Authorization,

already obtained recipients authorization.

2. Beth Israel in New Jersey to provide Recipient's insurance info.

and Authorization.

for NYPH/Cornell Donor.

Need direct e-mail(s) and phone numbers for all involved in the

potential SWAP Txp.

NYPH/Cornell Administrative staff would prefer discuss additional

financial agreements, etc.

with your Hospital Administration.
