As a new member, I'm actually heartened to see OT discssions taking place.

For me, it breaks up what can quickly become repetitious.

As to " ReplyAll " options, posts all replies to the forums into

digests. If a public discussion is taking place, everyone who gets mail or

digests will get all. The only other option is to take it off board, which,

IMHO, should only be suggested when a debate becomes a personal issue. If

everyone posted the same, used " netiquette " (hate that buzzword), and kept

on topic all the time, this group (and any other) would lose whatever flavor

and style it has, and people would really be bored to tears.

In the past I have had some success teaching pronouns in the expressive

format prior to introducing them in the receptive format. I would say, most of

the

children I have worked with learned pronouns as well as the appropriate

reversals with fewer errors when I started receptively.

First. I typically would like the child to have mastered the following

programs prior to introducing pronouns:

1. Body parts - since the program will start with identifying whose body

part.

2. Prepositions - since this is the first abstract language program I would

typically introduce. Also, for some reason, the kids I've worked with that

acquired and generalized prepositions quickly learned the pronouns quickly as

well.

3. Gender Identification: since the receptive component involves " his " vs.

" her "

If you decide to go the receptive route, try the following order:

SD1: " Touch your/my (body part) "

R: Touching correct body part.

Prompt: Moving child's hand from the elbow.

Mastery: All body parts randomly presented.

SD2: " Touch his/her (body part) "

R: Touching correct body part.

Stimuli: Pictures or other people besides the teacher.

Mastery: Generalization.

NOTE: Prior to introducing SD3, be sure to randomly present SD1 and SD2.

Typically, during the randomization of SD1 and SD2 any reversal errors the child

may demonstrate will present themselves. This would allow you to isolate the

error and systematically reintroduce it if necessary.

SD3: " Touch our (body parts) "

R: Touching both the child's body part and the teacher's body part

simultaneously.

Mastery: Generalization.

Again, be sure to randomize SD1, SD2 and SD3 in order to identify any

reversal issues that may be demonstrated by the child.

SD4: " Touch their (body parts) "

R: Touching other people's body parts.

Stimuli: Pictures or two other people besides the teacher.

Mastery Generalization.

Notes

1. Once SD4 has been consistently demonstrated, be sure to randomize it with

the previously acquired responses to identify any reversal issues.

2. Once the child has achieved mastery criteria, the receptive component can

be used to prompt the expressive components.

3. Depending on the age of the child and their ability sight read, I have

had success using textual cues as well.

Dear

HVs in Cheltenham & Tewkesbury PCT have recently started following a

core programme. It is based entirely on research evidence - which I can

get hold of if you're interested. The core contacts are: antenatal

visit, primary visit, 6 week maternal mental health assessment, 9 month

review and 2 year review. All these contacts are face to face, although

the 2 year review can be carried out by telephone if appropriate. There

are also benchmarks for each contact so that every client receives the

same level of care.

My understanding is that this is a minimum. HVs negotiate additional

contacts with each client according to need. Including an antenatal

contact is meant to cut down on weekly postnatal visits, as the HV has

invested time antenatally to develop a relationship with the

family/client so should need less time for this postnatally. In keeping

with Hall 4, HVs are encouraged to weigh babies less frequently, but HV

run weekly surgery based baby clinics still take place. The core

programme should allow HVs to free up more time to devote to public

health activities, such as running groups, but obviously this is going

to depend on the level of need within the caseload and also HVs'

motivation to branch out in this direction.

It would be interesting to see how much variation there is among core

programmes - equity of service might exist at a local level but what

about nationally?

I am trying to find what variations, if any, there are on the idea of a

Here in Haringey we are revewing our child health surveillance programme and our health visiting service together ( using Hall 4 as one of the tools) We have convened a steering group and a number of smaller working parties with health visitors, midwives, managers et al. Our core programme is as yet under negotiation!!

It is a huge piece of work and we are breaking it into small chunks. We have revised our health visiting records using the Frame work for the assessment of children and need and their families (DOH 2001). We are awaiting the evaluiation of a family health needs assessment tool form one of our local Sure Start programmes.

As part of all this we now have the opportunity to challlenge and health visiting boundaries ( we are still GP attached....and have health visitors travelling far and wide across the Trust). At last we are in the right arena to challenge the way health visiting has been configured over the years and hopefully move health visiting back into familiar territory....the LOCAL COMMUNITY. In theory they have never left it but its impossible to deliver anything stuck in your car in a traffic jam visiting a client who lives 5 miles away..who you are never likely to see again!

Its no small task but..but watch this space !

That's very helpful Jane, thank you. I guess I had kind of placed the child

health surveillance into the core programme in my mind. Is that unusual?

it into small chunks. We have revised our health visiting records using

the Frame work for the assessment of children and need and their families

(DOH 2001). We are awaiting the evaluiation of a family health needs assessment

tool form one of our local Sure Start programmes.

As part of all this we now have the opportunity

to challlenge and health visiting boundaries ( we are still GP attached....and

have health visitors travelling far and wide across the Trust). At last

we are in the right arena to challenge the way health visiting has been

configured over the years and hopefully

move health visiting back into familiar territory....the LOCAL COMMUNITY.

In theory they have never left it but its impossible to deliver anything

stuck in your car in a traffic jam visiting a client who lives 5 miles away..who

you are never likely to see again!

Its no small task but..but watch this space !

Sorry to come in to the debate rather late - have been away. Here in Bath all routine contacts after the 6/52 developmental assessment and routine immunisations officially have been stopped and a purely targeted approach instigated. However, most HVs regret this and are seeking to continue to offer their service universally and with families with older children. We are also working in a population public health way. Ruth
