We must seize every chance we get to let the public know about the reality of the front lines of care, to expose the gaps between top-down policy and what is likely to actually help us help our patients. Consider hitting the theme that real health care reform has to go beyond “insurance for all” to address the impossible burden of work demanded by insurance companies, a burden crushing our hopes and ability to deliver on our professional obligation to our patients. Consider making the point that “health information technology” does not currently perform many of the functions we need to deliver effective care. Without addressing how offices function, HIT fails to deliver the promised billions in savings. Consider making the point that the current NCQA PPC-PCMH™ tool is flawed and as likely exacerbate current problems as help, that we need additional ways to understand value than this 180 element behemoth, that in addition to the infrastructure and technology focus of the PPC-PCMH™ we need a measure set that values the fundamental attributes of effective primary care: “I can get care when I need it” “I have a provider who knows me” “My primary care serves the bulk of my needs” “My primary care coordinates my care throughout the health system” It is a travesty that we suffer a policy environment that pays only for “office visits,” rewarding procedures over caring, and places zero value on all of the work we do for our patients and families outside the “encounter.” It should be intolerable to the US public that the single solution to measuring quality in office practice – the NCQA’s PPC-PCMH™ - favors large institutions and mega-groups (Dianne Rittenhouse et al Health Affairs 2008). We cannot write off the 45% of practices in the US that are small and solo. Why don't we all create the forum for publishing new columns? Consider a public blog addressing health care issues which could also serve as a web ring that links us all together and increases our collective internet presence? Not sure if this already being done with the wiki and IMP locator, but I am thinking of a blog that is linked to some mainstream health related websites. We could have our groups members publishing articles to a public blog. It would kill three birds. 1) Get the message out to the public about what is missing and what is needed in the healthcare system. 2) create good P.R. For IMPs in general and each of our practices by pulling the public in and increase awareness of the success of the IMP model. 3) improve our rankings in search engines by hopefully creating more hits on our " webring " . Can anyone comment on whether this is a new idea, or been there done that. Stew.
