I have been billing office call for the bursitis diagnosis, and billing injection for the pain. So far have been getting paid. I even got half paid for a second charge the other day when I also injected the bicep tendon sheath along with the bursa.

I know I can bill insurance for a procedure along with an office visit if the procedure is for a separate problem, as long as I use a -25 modifier. But what is the rule when the procedure is for the same problem? IE, someone comes in with shoulder pain, I diagnose subacromial bursitis and inject the bursa. Am I only allowed to bill for the office visit or the injection, but not both?
