I think it's most important that you always do it the same way so that the

labs can be compared apples to apples instead of apples to chairs. IMO,

your doctor's preference should be respected, probably for this reason.

My preference is to have an early fasting blood test and take any and all

morning meds afterward.

I strongly recommend you do the testing prior to taking your thyroid in the morning. I spent the most miserable year of my life because i had an idiot doctor that didn't understand the impact of my having taken my thyroid prior to testing.  My T3 was well over range and the doctor decreased my dosage to a very miserable level. Once I got educated and started doing the labs before taking my thyroid, reality hit and it was clear I was way under medicated.   It's really impossible for you to know exactly how your body is metabolizing the T3 unless you do several blood tests during the 4-6 hour time period.  Based upon how I feel, I believe my T3 peaks far sooner than 4 hours .  So, I personally would follow the nurse's suggestion. There are doctors purported to be able to extrapolate in cases where you have taken your dosage, but I think that is quite rare.
