Please go to this site and take the survey as soon as possible, if you have not done so already. Below is a list of states from which have still not received completed surveys from. If your state is listed here, then I urge you to go to this site and complete your portion of the survey. It will take approx 10 minutes of your time. The questions are mostly multiple choice, all of which are quite easy to understand and to answer.
