2728 Forms -- ESRD Network

According

to our ESRD Network contact (New England), it is the center monthly report to ESRD noting on the event

records that a dialysis patient has transferred in for transplant that month which

triggers notification of a transplant for a patient with an active 2728, thus a

modality change for the patient. ESRD enters this information into a

system accessed by Social Security for the transplant information to be

available with Medicare.

Our ESRD representative indicated that the

initial 2728 is an active document throughout dialysis, transplant, and the 3

years after transplant. She added that for patients who had an initial

2728 submitted (from dialysis or transplant as 1st modality of ESRD

treatment), a subsequent 2728 is completed only if 1) dialysis patient has had

recovered renal function, been off dialysis for 12 months, and then restarts

dialysis, or 2) patient transplanted and has graft failure more than 3 years

post transplant so either starts dialysis or has a new transplant.

Medicare denials for immunos

Good morning, 2 of our local

pharmacies are suddenly having trouble w/denials from Medicare for

immunosuppression - the message is " no transplant on file " - and

in fact there is not transplant date listed in the CWF but

that's not unusual. All of these transplant recipients were on long term

dialysis so we are not required to file a new 2728 - any insights?
