2728 forms

Hi, I am in central PA, and we do have to fill out a 2728 form for

each transplant and mail into what is called Network 4 here. My boss

gets a report and it has to match up to how many transplants we do,

and which patients were on dialysis or not, at the time of

transplant. I fill out the patient information portion, pass it on to

the pre nurse who fills out medical/clinical part and has the

physician sign and return to me. I keep a record of all that we send

out.
