Lou,

I would be happy to answer that. In the days of

capitation, insurance companies did not pay anything more for doing biopsies,

excisions. However, malpractice in NJ for minor surgery is about 50-75%

higher than without minor surgery so after analyzing what I actually got paid,

vs. what it cost me to carry the malpractice, it was a business decision to

give it up. After all, why do it for free when someone else will get paid

nicely for doing it. Not even free, at my expense.

As far as joint injections, never taught in residency. Was

in a residency of turmoil and transition with little faculty. Wasn’t

something I wanted to teach myself. I am of the see one, do one, teach

one philosophy but got to get that hands on practice. The orthopods we

rotated with spent most of their time telling us how we shouldn’t be

doing any of that stuff as only they were qualified.

perhaps it is my imagination, but I am curious

as to why it seems that

many of the physicians on this list do not do procedures such as

routines skin procedures(punch biopsies, lesion removal etc.) as well

as joint injections. These are both very easy to do even within the

confines of a solo-solo practice. They also require minimal equipment.

Lou, I do all these, endometrial biopsies, I & D abscess, removal of skin lesions, including 2 melanoma's in last 3 yr. I trained on West Coast, we were taught to do everything, including OB, and forceps deliveries. Another geographical area difference, and perhaps an age difference, I have been out of residency now for 18 yr, I taught for 8 of those years at a family medicine residency.

I my capitated contracts in the 80's, I did get paid for all of the extras like

biopsies, even chest x-rays etc. The routine stuff was covered under the cap,

everything else was fee for service. Worked well.

I do lots of this, also do lots of ortho, since I live in a town with lots of

skiing, hiking and mountain biking. Pays well.

So . let's help the other guy. We have a variety of trainings but may perhaps as IMPS share more . This was done at CAMP this year and I hope we continue to do it We should try to add to and extend our skills as much as possible This serve s our patients well and serves the system in prevention of unwarranted referrals

My own residency has developed some rather humorous but useful teaching models to demonstrate Lps endometrail bx etcThen there are always those funny slippery diaphragms to fit. If a procedure can be learned , can b e billed for, an is useful to have in our quiver, then lets teach each other what we might not have gotten in resdiency.

Let's make a l ist for IMP camp: shoulder injections?Knee injections? delivering babies with forceps...( joke)Now, who wants to teach?

Happy to teach any injections, casting, forceps deliveries, endometrial

biopsies- but haven';t done any in 3 years. Also lesion removal, biopsies

whatever, even how to take xrays.

I agree

with Lou. Sending patients to specialists to do joint injections makes no

sense. As for the pain on injection, I have had numerous patients tell me that

the injection pain caused by my expertise is way less than that caused by my local

ortho colleagues. One elderly patient stated, “You

have the hands of an angel. I will never let anyone else do my steroid

injections again!!” Injections are easy and should be relatively painless---particularly

when performed by a caring individual within the realm of a healthy

doctor-patient relationship.

Re: procedures

I strongly disagree. This is the conventional claptrap fed to Family

Medicine by some specialists. If you have an interest in doing the

procedure-you have a willingness to learn to do it correctly and you

have enough opportunities to keep up your skill, then you should

proceed.

I’m not into doing procedures –

don’t care for doing biopsies, injections, suturing, etc. So just a

I have to agree, joint injections shouldn't be painful, and patients often tell me that I did a better job than the specialist who did it before. Willing to teach joint injections at camp, better, perhaps we should make a list of procedures wanted to be taught by someone, then match up people with the skills, who will be there. I think joint injections and Botox have to be about the easiest things there are to do.
