In the days of capitation, insurance companies did not pay anything more for doing biopsies, excisions. However, malpractice in NJ for minor surgery is about 50-75% higher than without minor surgery so after analyzing what I actually got paid, vs. what it cost me to carry the malpractice, it was a business decision to give it up. After all, why do it for free when someone else will get paid nicely for doing it. Not even free, at my expense. As far as joint injections, never taught in residency. Was in a residency of turmoil and transition with little faculty. Wasn’t something I wanted to teach myself. I am of the see one, do one, teach one philosophy but got to get that hands on practice. The orthopods we rotated with spent most of their time telling us how we shouldn’t be doing any of that stuff as only they were qualified. I trained on West Coast, we were taught to do everything, including OB, and forceps deliveries. Another geographical area difference, and perhaps an age difference, I have been out of residency now for 18 yr, I taught for 8 of those years at a family medicine residency. This was done at CAMP this year and I hope we continue to do it We should try to add to and extend our skills as much as possible This serve s our patients well and serves the system in prevention of unwarranted referrals My own residency has developed some rather humorous but useful teaching models to demonstrate Lps endometrail bx etcThen there are always those funny slippery diaphragms to fit. If a procedure can be learned , can b e billed for, an is useful to have in our quiver, then lets teach each other what we might not have gotten in resdiency. As for the pain on injection, I have had numerous patients tell me that the injection pain caused by my expertise is way less than that caused by my local ortho colleagues. One elderly patient stated, “You have the hands of an angel. I will never let anyone else do my steroid injections again!!” Injections are easy and should be relatively painless---particularly when performed by a caring individual within the realm of a healthy doctor-patient relationship. I do all of those. perhaps it is my imagination, but I am curious as to why it seems thatmany of the physicians on this list do not do procedures such asroutines skin procedures(punch biopsies, lesion removal etc.) as wellas joint injections. Willing to teach joint injections at camp, better, perhaps we should make a list of procedures wanted to be taught by someone, then match up people with the skills, who will be there. As for the pain on injection, I have had numerous patients tell me that the injection pain caused by my expertise is way less than that caused by my local ortho colleagues.
