E-visit set up and phone systems

Finally getting down to the nitty gritty of starting my IMP. target date to start will be Oct 1st. Just reviewed the IMP wikia. It's very helpful. Questions-

1. What was missing from wikia was how to set up an E-visit. I know there have been various emails from people on this list serve but not easy to locate thru these 1000's of emails. Can Lynn or anyone else send me a link with step by step intructions? For those doc with different fee schedules for different pts, do you set up Evisits with different fee amounts? for example, Plan A pays.., Plan B pays.. Maybe getting ahead of myself.

2. What phone system do you use? I was thinking of Vontage or something cheap. I plan to use onebox. Love the idea that voice mail can go to email/text.
