Guest guest
Hi , I included them with the packet I sent in. The office should have provided you with a checklist of the items they want. You could call his office in the a.m. and ask for sure. Kristy Medical Records Can anyone tell me if Dr. Goldberg wants to see OT, PT and speech assessments prior to the initial appointment along with all the medical records? Guest guest
Hi- We included all of his most recent evals OT, PT, Speech, SpEd, as well as his complete Dr. records- (psychologists, neurologist, eye drs, pediatricians, allergists, etc.) in our initial packet. It was quite a large file, but worth all the time and money to copy and overnight it:) Jenn -- In , Cheryl Lowrance <c.lowrance@...> wrote: > > I didn't include them in ours. Guest guest
He would be interested in the OT or PT if there are hypotonia issues or other soft neurological signs that could be the result of something other than 'autism'.
