Things were going pretty well and the doctors took me off my beta blocker Bystolic. I was tapered from 15mg to 10mg and then finally 5mg of Bystolic over a 2 week period. During the last two weeks on the 5mg of Bystolic, my blood pressure averaged 115/77. I didn't have a reading over 130/80. Since stopping the 5mg of Bystolic, my blood pressure has been higher. My average readings were 136/85. I was feeling good and continuing to DASH and eliminate sodium from my diet. Over the last month I have been more active than I have ever been and have maintained a weight of 189 lbs. I weighed more than 235 lbs last August. My blood pressure the last week has been very high. My average over this time has been 144/95. I am still DASHing and eating low salt. I walk at least 45 minutes per day. I go to the doctor next Wednesday, and the plan is to increase my dose of Spiro to 50mg. I am concerned that doubling the dose means that my PA is getting out of control. Dr. Grim, you mention that AVS is not necessary if DASH and spiro is working. Since my BP has been over 90 on the low number, does that mean spiro is not working? I was very excited to read o's success story about DASH and dropping the spiro dose. However, it seems that strict DASH and exercise have not prevented doubling the dose of spiro. 50mg might seem like a very small dose of spiro to some. However, the concern is that it is necessary to double the dose after only 6 weeks. I'm terrified about what high blood pressure will lead to eventually and feel like I am doing everything I can to stay healthy. It is truly frustrating when a 110% effort leads to a doubling of the spiro dose! I feel like I am only delaying the eventual heart attack that PA will cause. By stopping the Beta blocker it more then likey changed heart rate. > > > I started on Spiro 25mg about six weeks ago. Guest guest
I will request the urine tests on Wednesday. I take BP with a home digital monitor. I had it checked for accuracy 6 months ago at the doctors office. I will plan to bring it with me on Wednesday and mention the urine tests. My PCP really doesn't have a clue about PA. It concerns me that I can sense in her voice that she is worried. I am her only patient with PA. She wanted to put me back on Bystolic, but my endocrinologist at Wash U. told me that increasing to 50mg of spiro will be the plan if the BP is not down by June 8th (3 weeks after stopping the beta blocker). The endo at Mayo that I see was in agreement via e-mail and thought that the Bystolic could raise my BP for 4-6 weeks after stopping it. Is there any danger in increasing spiro to 50mg? If my BP is only up due to stopping the Bystolic, can 50mg cause a BP drop to a dangerous level? Could the 50mg of spiro cause an emergency level high potassium? I average between 4.0 and 4.5 k on 25mg of spiro currently. 34 yo male. Diagnosed 4/11. Spiro 25mg. CT scan does not show an adenoma, but does show left adrenal " lumpiness " and larger than right. > > > I started on Spiro 25mg about six weeks ago. Guest guest
My heart rate is getting better but is still very sporadic after the beta blocker. The last three weeks it has been over 90 at rest. It was under 60 on the beta blocker. This week the resting rate is around 70, but my blood pressure is higher than ever. My heart rate was shooting up to over 110 when standing up last week. This week things seem to be improving and I don't go over 100 when standing. However, I only wear my heart rate monitor before going to the gym, so I only get a small view of the day. The heart rate would be in line with what the Mayo doc predicted regarding Bystolic. He is not concerned about the BP at this time, but still wants to increase spiro to 50mg if it does not come down soon. If 50mg of spiro is not really going to harm me, it is probably best to increase it until my BP gets back to normal. I personally have no idea and wish I had attended medical school so I could keep up with all of this! > > > > > I started on Spiro 25mg about six weeks ago. Guest guest
Dr grim says to disregard the first reading. Is this what most of your readings look like? Guest guest
Most of the readings look pretty similar to today's scenario. The second reading is usually 7 pts lower than the first. 128/86 is still high though right? Guest guest
I've heard this from other doctors as well. At Mayo, they take 6 total blood pressures. The first one is thrown out. The other 5 are averaged. It is a machine that is in all of the internal medicine and cardiology offices. The funny thing is when the nurse manually took my BP it was 145/92 at Mayo. The machine averaged 119/78. Anxiety plays a big role for me. The cardiologist ignored the manual BP and went with the machine average. > > > > > > > > > > > > > I started on Spiro 25mg about six weeks ago. You go from sitting to standing to walking to where B/P is taken. It is always going to higher then when at rest. I think they should have you sit for about 10 min before taking B/P. The cardiologist should have taking his own B/P reading on you. > > > > > > > > > > > > > > > I started on Spiro 25mg about six weeks ago. Dr. Grim, you mention that AVS is not necessary if DASH and spiro is working. Since my BP has been over 90 on the low number, does that mean spiro is not working? Guest guest
I've always thought that was a crazy way to do it - after you've been in the waiting room (in my experience, usually far longer than you should have been since the docs are always running late!) they call your name, you get up and move quickly, and then the next thing you know they're taking your BP. Not to mention the whole " white coat syndrome " thing too. My BP at home is consistently around 138/90, but the minute I enter a doctor's office it's consistently 160/100. I've had my meter checked and re-checked, and it's accurate. I've learned that a good HTN doc will check it several times during the appointment. My nephrologist checks mine both on the arm and on my leg - he's the only doc I've ever seen do that before. > > > > > > > > > > > > > > > > > I started on Spiro 25mg about six weeks ago. Guest guest
By this chart if you didn't have PA you would have Prehypertension. Guest guest
If you have had HTN the home goal should be below 135/85. Unless u have DM OR renal disease. You might want to download JNC 7 and read it cover to cover. Tiped sad Send form miiPhone ;-)May your pressure be low!CE Grim MDSpecializing in DifficultHypertension Most of the readings look pretty similar to today's scenario. The second reading is usually 7 pts lower than the first. 128/86 is still high though right? The bottom number is higher than I would expect it to be if I was on the right dose of spiro. > > > > > > > > > > > > > I started on Spiro 25mg about six weeks ago. Not to mention the whole "white coat syndrome" thing too. I'm terrified about what high blood pressure will lead to eventually and feel like I am doing everything I can to stay healthy. It is truly frustrating when a 110% effort leads to a doubling of the spiro dose! I feel like I am only delaying the eventual heart attack that PA will cause. > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > > >
Guest guest
Hi . I orginally started out with 100 mg spiro in October ( 50mg am/50mg Pm). That keep potassium in check and blood pressure controlled for a short time. Six weeks later I was in the hospital again for low potassium (2.7). We then increased it to 100mg in the AM and 100 mg in the PM. Within two weeks my liver enzymes skyrocketed. We then adjusted to 100mg in the AM and 50mg in the PM which is what I am currently take. I bounce between 3.9-4.4 on the K. Then my blood pressure started skyrocketing in between all of this and we added in 5mg Bystolic. Since DASHing and Weight Watchers the blood pressure is way low. I just spent the last 3 weeks weening myself off the bystolic. My readings were 90/50 which made me very lethargic. Now I am averaging 110/75 and feel pretty good. In the doctors I have been averaging 130/90 but I just posted on another email that the car ride raises my blood pressure so I am not too worried about that higher reading. Your doing everything right so don't give up!! You may just have to find the right dose of spiro for you. Kellie Female – Age 41 – Height: 5’5” – Weight since HTN and Low K at all time high of 190.Treated for HTN from age 18-32 when it suddenly dropped. Started HCTZ for swelling in 04’. Have not tested positive for PA however the treatment of Spiro is keeping both HTN and K in check. No tumor on adrenal gland. Insulin resistant-Hypoglycemic-No testing of AVS. Meds: MRI revealed benign tumor in left eye socket Meds: 100 mg of spiro in the morning and 50 mg in the evening. Xanax: .5 mg three times a day. DASHing in conjunction with Weight Watchers I started on Spiro 25mg about six weeks ago.
