However the LA still has a duty to a young person with SEN until they are 25 - but there isn't a Statement any more, so things get even more difficult. Connexions ought to help you here. Guest guest Posted December 1, 2010 Report Share Posted December 1, 2010 This is the pdf of the section from the SEN Toolkit about Trtansition planning: http://www.teachernet.gov.uk/_doc/4615/Toolkit%2010%20Text.pdf should have had a transition plan since the annual review in Year 9 and it should have been updated annually.
