Are they doing urine tests also? Will be interesting to see where your body

thinks you are. You once asked where my BP was before Spiro and it was high

160s/87-90. Dr. G. will probably chime in but you may want to go to medline and

check side effects and overdose info. Hopefully your body will adjust quickly

and you will see positivw results soon.

- 64 yo morb. ob. male - 12mm X 13mm rt. a.adnoma with previous rt. flank &

testicle pain. I have decided against an adrenalectomy at this time since

Meds. are working so well. Current BP(last week ave): 123/69

Other Issues/Opportunities: OSA w Bi-Pap settings 13/19, DM2. and PTSD.

Meds: Duloxetine hcl 80 MG, Mirtazapine 15 MG, Metoprolol Tartrate 200 MG,

81mg asprin, Metformin 2000MG and Spironolactone 50 MG.

No he has not ordered urine tests.  I did ask for one and he didn'tthink it was necessary right now, but he does want the blood tests.Your diastolic prior to Spiro at 87/90 would have been a wonderfulreading for me and I don't think my Dr. would have considered it

as significantly high....however, the 160 systolic would be.The only reason I have not been checking on the side effects ofSpiro is because I did not want to be influenced by what I read.Sometimes, if you know of a side effect ahead of time, you just

might manifest it.However, through reading various e-mails, I have found out whata few of them are.I see that you are taking Metoprolol.  This was one of the very firstBP meds I was put on many, many years ago.  I took it for quite

a while but it gave me severe nightmares and did very little asfar as reducing my BP.It is still early days for me and I am looking forward to seeing greatimprovement as the weeks and months go by.

If he does not check the urine Na he has not way of knowing if the patient is following his recommendations. The next step is to add BP drugs. Thanks Dr. Grim:I will bring up the subject of a regular urine test with the Dr.on Thursday.Also, in your opinion, what would be the first BP med. toadd to the Spiro?  I know that various types will have no effect

on lowering the BP.  I will check out the studies on side effects comparing the group taking the Spiro and the group on a placebo.How does the blood Na differ from the urine Na?

Jan, some of the symptoms you describe are merely an anoyance (providing you

have a good book and remain close to a facility). Others are listed further

down under the catagory, " If you experience any of the following symptoms, call

your doctor immediately: " If you don't want to read the symptoms yourself for

fear of becoming a " cyberchondriac " , just do me a favor and call your doctor in

the morning and read your original quote to her/him!

I never have had a doctor tell me all to look out for and I understand the drug

companies may err on the side of caution to lessen lawsuits but there must be a

reason, no matter how small, that they list them! Remember, I am the one who

had an adrenal adenoma seen in a ct-scan 6+ yrs and in a lab report 3 1/2 yrs

before anyone bothered to tell me " because it was no big deal " . I ask questions

now and tell them if they give me the wrong answer! (And BTW, it could have

been a " big deal " if LVH had of progressed much further!)

I'll leave that there and simply wish you good luck! Your doctor might have

been real happy with a diastolic pressure of 87-90 but remember, I have DM2 so

my goal is 130/80! (And my doctor is real happy at 123/73!)

Dr. Grim, I feel you are doing us a big disservice with your answer for us to do

our own research on drug side effects. If that's the way it should be why don't

we just research, DX and self-medicate! And to imply we will become

" cyberchondriac " will just cause some to stifel their thoughts and the whole

group will lose valuable input. If we are going to help each other, we need to

see the whole picture and that is why I tend to say whatever in HE_ _ I want!

The best example I can use right now was my LVH correction and your explaination

of how the system works from HTN to the lungs. I hope other's took that to

heart, pun intended, and realized other symptoms this might have an effect on!

(Had I been bashful or stifelled I would just have gone on to my doctor,

exclaimed " oh wow " and we both would believe in magic!)

As to where and why I look up meds. I go to Medline Plus because that is where

MyhealtheVet takes me when I want to look up a med, an action they encourage!

Not all side effects are listed on medline pluse or drugs.com. If you really

want to know about drugs get a copy of the Physician's Desk Reference. An other

book is " The Essential Guide To Prescription Drugs " By J Rybacki Pharm.D

Medline Plus is probably accurate enough for my purpose. I always know where it

is and they will update it for me so it will be more current without me having

to spend extra $$$!

I just reread your post and see you mentioned metoprolol and if you didn't catch

my posts under beta blockers, I think it is a leftover from the 7 BP meds I was

on. I expect the removal was an oversight and its use in PA is probably between

nil and very limited (I'm not a Dr. so I may have that wrong but I saw your

question to Dr. Grim and I'm pretty sure that is not the answer!) Actually it

probably won't be an ACEI or ARB either since they don't work w/low renin. I

won't guess further although the HTN Primer gave me some ideas - don't want to

be practicing w/o a license!

- 64 yo morb. ob. male - 12mm X 13mm rt. a.adnoma with previous rt. flank &

testicle pain. I have decided against an adrenalectomy at this time since

Meds. are working so well. Current BP(last week ave): 123/69

Other Issues/Opportunities: OSA w Bi-Pap settings 13/19, DM2. and PTSD.

Meds: Duloxetine hcl 80 MG, Mirtazapine 15 MG, Metoprolol Tartrate 200 MG,

81mg asprin, Metformin 2000MG and Spironolactone 50 MG.

Thanks .  I have ordered the Hypertension Primer from Amazon and expectto get it within two weeks.  I am looking forward to becoming more educated.I don't want my Dr. to put me on a BP med. that is not going to work for folks

with PA.  Perhaps Dr. Grim can tell us the family of BP meds. that would bethe most appropriate and then I can casually mention it to the Dr.  I know thatthey definitely don't like to be told what to do, as you can hardly blame them.

Since many with PA have heart rate problems and beta blockers somewhat control

them and for non PA they lower B/P. Mostlikey given by DR that think PA is

something they think they will never see.

Just take your new HTN Primer in opened to chapter C167, lay it on his desk and

ask him when was the last time he read that one! (If he wants to know what

other drugs, open it to page 566 1/3 of the way down in second column labeled,

" Other Drugs " ! If he gets messing around with your drugs make sure he keeps an

eye on your K level! (Mine was checking every 2 weeks when the Neprologist had

her adding an ACEI before I pointed out that it wouldn't work!

How much experience does your doctor have in treating PA? I'm treated by

doctors at the local Veterans Hospital (VA) and have ment very few that really

knew much about it (maybe 2 Neprologhists out of 4 and 1 Endrocologist.) Many

of the specialists rotate between the VA and a local teaching hospital (Darmouth

in Hanover NH). It's unbelievable how little they know and they all want to

treat you as if you have regular HTN which does not work! I was lucky that I

have a PCP who believes me and Dr. Grim through me and wants to learn. I really

think she is almost as excited as I as things come into control! (She has been

my PCP so she has shared my frustrations and knows how emotional one can get

when nothing works and the weight keeps going on. I finally told her, as I was

approaching 320 lbs, that I would " would wring the next young whipper snapper's

neck if they mentioned weight loss! " ) She can't believe that I have lost ~25

lbs, BP is under goal for DM2 (75% of people w/DM2 can't hit goal), need for

suppl. oxygen is gone and mood & attitude! In fact she asked my permission to

keep me on a 2mo recall so she " can continue to be my #1 Cheerleader!) That's

her quote, I would probably be on a 6mo recall normally!

And by the way, I do blame them if they don't know and don't want to be told if

I have valid information that will further my treatment! I may not have their

medical experience but I bet they don't have my troubles shooting and

researching capabilities using a computer including a 27 year professional

career! I guess the bottom line is I am not a Lab Rat and this is not a new

path - follow the rules and we will get this thing licked! I won't rant any

more but I never had time for programmers that guessed how to write or diagnose

a program and I don't have time for doctors who don't follow the rules and seek

assistance if needed, not to say that there aren't times they may have to try

different things but they had better have tried the standard procedures first!

DM2, CKD Stage 3, COPD (emphysema) in addition to PA. I would prefer they KNOW

instead of THINK! Sorry if I appear insensitive but it is my ticker they are

I think at least in my case and many others If Dr dosen't think about PA and SX

we are given meds that cause more harm then good.

I am sure BB have there place in treatment of heart related problems. Just not

in someone with possable PA.
