There are some very good Dr at Dartmouth but also some not very good ones.

I can relate to not wanting to travel when you don't feel well.

Also have to think about traveling after having surgery. Seems some have a lot

Thanks Val, I will. When I looked for a Dr. Bravo, the first I found was a

" Cesar A Bravo, MD Geriatrician, Internist, Nephrologist " who appears to be a

well respected doctor in the Houston area with over 40 years experience. The

last listed speciality seemed to fit but I keyed on the first! Since we had

been discussing a 40 year old check I wasn't ready to put my bet on a doctor

whose speciality was/is Sr. Citizens. I may have been a little too forceful

with my question and I appologize but I have been Harping on know your source!

As for he being " smarter than all of us combined " , speak for yourself, I'm

pretty damn smart depending on the subject! See how he is at talking to a

computer at machine level, making snow, driving a 45' tour bus all over e. U.S.

& Canada, running a state level youth hockey program, milking a cow, surviving

on the food you grow and raise (in fact all the eggs we use come from the two

hens in my back yard!) I will concede the Dxing and Rxing to him as long as I

reserve the right to question him - a Dr. C. E. Grim taught me that was right

and prudent!

Really? Most of Europe has public funded health and still plenty of surgery,

including adrenalectomies.

You have put Cleveland Clinic in the search as well.

I would guess that not all can take meds. Some have to have both glands removed

as that seems to be the only way to lower B/P. Not sure if that is the better

choice.

Francis, I was hoping I wouldn't have to guess when I had a well respected

doctor with over 50 years experience to draw on. He is obviously in sync with

Dr. Grim most of the time but I got the message that sometimes he recommends

surgey. I thought it might be good to tap into his vast experience before it

is lost to us forever, afterall he must be well into his 70's or more.

If we are going to start guessing let me throw my guess out as to why there is a

40 year old check. My bet is insurance is the answer after they analized their

claims! They found a vast majority (I'll pick atleast 97%, you pick your own

number but it must be over 95%) went from AVS to Surgery if PTN was under 40.

They then said why pay an extra $25,000 when we are going to operate anyway. So

what if we are wrong 3% of the time, it is only money and we don't have to live

with it! Do the math, $25,000 * 97 = $2,425,000! What does it cost to do 3

unecessary adrenalectomies! Looks like you just saved 2million and maybe even

had enough to pay your lobbyists!

Any other guesses?

- 64 yo morb. ob. male - 12mm X 13mm rt. a.adnoma with previous rt. flank &

testicle pain. I have decided against an adrenalectomy at this time since

Meds. are working so well. Current BP ave. 112/60.

Other Issues/Opportunities: COPD w/ft Oxygen, OSA w Bi-Pap settings 13/19, Type

II Diab. and PTSD

Meds: Duloxetine hcl 80 MG, Mirtazapine 15 MG, Liothyronine 5MCG, Methadone 15

MG(titrating off S-L-O-W-L-Y) , Metoprolol Tartrate 200 MG, Metformin 2000MG,

Lisinopril 2.5MG and Spironolactone 50 MG.

A bit closer look at the Numbers from my other posting

DATA SYNTHESIS: In 37.8% of patients (359 of 950), CT/MRI results did not agree

with AVS results. If only CT/MRI results had been used to determine

lateralization of an adrenal abnormality, inappropriate adrenalectomy would have

occurred in 14.6% of patients (where AVS showed a bilateral problem),

inappropriate exclusion from adrenalectomy would have occurred in 19.1% (where

AVS showed unilateral secretion), and adrenalectomy on the wrong side would have

occurred in 3.9% (where AVS showed aldosterone secretion on the opposite side).

OK 14.6% of the time AVS seems to be better then CT for bilateral problem. So

in this case you don't have AVS but have adrenalectomy. Would still have PA.

19.1% of the time CT doesn't show any thing but you have AVS and have

adrenalectomy based on AVS and possible cure.

3.9% of the time CT shows tumor on wrong side you don't have AVS but have

adrenalectomy that removes good gland and you still have PA.

Thanks! That does sort of explain the 95% plus figure being quoted at me

although I had thought that included bilateral problems in the 5% too. Glad

M got her AVS - she must be in that minority.
