This is the report from when I saw Dr Bauman for the adrenal adenoma. At the

time I saw him I had blood tests where my K was

3.2 and many B/P readings over 140/80.

pt w/ 18 mm mass in left adrenal

no Sx's of HA, no spells, no nervousness or tremolous episodes

no DM Hx aunt w/ DM

HTN treated for 3 yrs but ?elevated in past

nl K A & P

Pt w/ incidental small adrenal adenomas found on CT.

Neg cortisol w/u and nl glu and K. No Sx's of pheo.

Given the small size and lack of labs or Sx's, I

would not proceed to any other testing at this time.

This is the report from when I saw Dr Bauman for the adrenal adenoma. At the time I saw him I had blood tests where my K was

3.2 and many B/P readings over 140/80.

pt w/ 18 mm mass in left adrenal

no Sx's of HA, no spells, no nervousness or tremolous episodes

no DM Hx aunt w/ DM

HTN treated for 3 yrs but ?elevated in past

nl K A & P

Pt w/ incidental small adrenal adenomas found on CT.

Neg cortisol w/u and nl glu and K. No Sx's of pheo.

Given the small size and lack of labs or Sx's, I

would not proceed to any other testing at this time.

All testing was done at same VA. Like I said he is a 5 min wonder see him for 5

min. and wonder why.

There was a lot in my recorded about my uncontroled B/P he could have looked at.

The only other place that have been to is Dartmouth. Because the VA is part of

the Dartmouth med school VA Dr can access medical records from Dartmouth.

Dosen't work the other way unless the Dartmouth Dr also works at the VA.

My Dr has put in my record many times about uncontrolled B/P.

BP readings that have been as high 199/100 K drop from 4.3 to 3.2 after being

started 25 MG of HYDROCHLOROTHIAZIDE fulid retention Dizziness Fatigue

Tachycardia shortness of breath brain fog 2 cm tumor on left adrenal gland most

urine test show low SP.GR

2.2 cm low density lesion mid pole left kidney greater than expected for a

simple cyst.

meds I now take and was on for the two PRA ratio test. PAR ratio wasn't high to

DX PA

ATENOLOL25MG TRIAMTERENE50MG POTASSIUM CHLORIDE 20MEQ

FUROSEMIDE 60MG TAB.

I added my information from the other posting to this one.

Until I can get a Dr that will work with me on DASHING I feel there could be

problems with it at this time. If I don't have PA what I read about triamterene

is K can get high if you eat a lot of K. Also being on 60MG of furosemide not

sure how much to cut back on salt.

If your BP and Sx are not well controlled on current Rx then would consider a

trial of spiro based on the CT and Hx of low K.

But I think you have had a problem getting VA to do this.

If your BP etc is doing well then I would sit tight and keep DASHing.

How long have you had HTN?

Has DASH helped?

Dr Grim You might write to to the VA about working with them. From what I can

see Secretary K. Shinseki is trying to do many things to inprove the VA. He

might just listen to you. I would add his name to this address.

Here are my thoughts on maybe some contact from you might be a good thing.

It would be my guess that since Dr Trask listing at Dartmouth doesn't state she

is board certified in Endocrinology but is listed as being part of the

Endocrinology team she is in there on the Fellowship program.

From my visit to the Dartmouth Endocrinology team and having both the one of the

teaching Dr and Fellowship Dr look at me and the teaching Dr telling both me and

Fellowship Dr that the Meds I am on do not affect the tests for aldo and renin.

You have a chance to make sure one less Dr will be taught to not do testing

wrong.

About that time I pulled out Dr. Grim's paper and laid it in front of her. She

actually exclaimed, " Hey I know of him, I have read some of his papers! " I told

her that he asked that we provide a copy to all our treating Drs. and that I

belong to a group that he moderates and that I had highlighted it in red in the

paper. I invited her to join!

Seems she has some interest in you work. The best teaching tool is this group

Maybe a note from you will get her to join and she will become a Dr that all of

us from northen New England can go to.

Hope she does it would be a big asset to WRJ VA and DHMC and may even give her

career a boost since she appears to be relative new at this location! I primed

her for you but will give you the feather if we only get one.

- 64 yo morb. ob. male - 12mm X 13mm rt. a.adnoma with previous rt. flank &

testicle pain.

Other Issues/Opportunities: COPD w/ft Oxygen, OSA w Bi-Pap settings 13/19, Type

II Diab. and PTSD

Currently on Spiro. 50 MG and titrating last 2 BP meds preparing for

adrenalectomy, maybe. Current BP ave. 112/60. (135/72 today as they prepared to

pull a tooth!)

Meds: Duloxetine hcl 80 MG, Mirtazapine 15 MG, Liothyronine 5MCG, Methadone 15

MG (recently reduced by 5 MG), Metoprolol Tartrate 200 MG, Minoxidil 5 MG,

Metformin 2000MG, Furosemide 40 MG and Spironolactone 50 MG. WE eliminated

Oxycodone/Acetam 10/650 MG 10 days ago and doubled the Metformin .
