Hello Everbody... Ortostatic Hipotension :S

My Information:

Obese (BMI 31.7-it used to be 35.5 ) , PA , Agoraphobic.

Meds: 25 mg of Spiro, 1mg of clonazepam.

HI, well I got my PA diagnostic last year on November, and before that I had

ortostatic hypertension, ok now that I got my diagnostic I started Dashing and

I'm on spiro ( I started at 50mg of spiro, then moved to 37.5 then to 25 )

anyhow with the Dash and spiro the problem is now that my BP is low, 110/70

while in bed, when I stand up the systolic can be heard at 100 but we are not

listening to the diastolic correctly we think it stops on 70 also but the sound

is so faint it's hard to accurately meassure it.

now whenever I stand I get that bp of what seems to be 100/70, but with a LOT,

of Tachycardia, while I'm in bed my HR is 60-70 when I stand up depending on the

day I can go as high as 110-120, so really I'm almost doubling, and keeping a

low BP, of course I'm getting dizzy.

Today I'm gonna start at half the dose of spiro, 12.5 mg a day to see if this

helps, but really this is hard.

I have no idea because I have not found any biography but when I stand up and my

HR is that high and my BP is low if I try to make exercise my HR goes insanely

high, and my BP well does not change that much, even with very low intensity

exercise... ( I'm talking slow tai chi here )

Now a few times I have had PVCs and last week I did about 2-3 hours of bigemini

( 1 beat normal, 1 PVC ) it was scary, but I had done it before.

Another interesting fact is that I can do stationary bike as excercise, my HR

does not go up, and my BP stays put, so I'm not on such a bad physical

condition.

I need advice on what to do I can't stand the fact that every single time I

stand up I get dizzy, I even bought a cane, to walk around, so this is not

really helping me with my agoraphobia. ( cause I go out, I feel dizzy , I think

it is anxiety, so I go back in the house to my bed )

Now I Have to say I'm a bit scared of lowering my Spiro to 12.5 because at 50mgs

a day 2 days I had a small hypertensive crisis in the mornings, one day I woke

up with a small headache and I had my by at 130/90 it kept going up to 170/100,

then it came down.

The other day it was 150/100 or so, so really I have no idea if I'm staying

unprotected if I do that.

And I have no clue how my BP will behave at 12.5mg of spiro, I think it will be

ok because even after 15 hours of taking 25mg my bp is 110/70.

but it scares me the crisis I spoke about before.

Just to show that I think my BP meassurements are fine, I have 3

sphingomanometers, one wrist digital, one aneroid, and a mercury column one.

I have had professional training when I was in medschool taking bloodpressure. (

3 hour physiology laboratory on Blood pressure taking, korokov sounds, cuff

placement, aneroid, mercury, digital, differences between arms, blocks, etc )

And I trained my wife to take BP as well, and we did the tests as blind, I

listened to the BP and I told her here and here to get ths sitolic and diastolic

and she read the sphingo, then we did it backwards.

and we double checked it with different apparatus.

So it's seems the meassurements are fine and they have an error of around +-

10mm/hg max.

Anyhow, I need help to see what to do, I really want to stand up and do more

excercise, thank you all ! :D

More concerning is the fact you have symptoms of dizziness when you stand or go out that goes along with the change in vital signs. If you are having that drop in blood pressure - and it is correlated by an increase in the heart rate - then likely something is amiss, it could be the medicine, it could be dehydration, or other things. But your body is talking to you for certain and may be worth digging deeper.

You know, when I first started on Spiro (late last year) I went through that a bit with the high heart rate and low blood pressure (I even asked the group if they had similar symptoms) and even fainted once putting me in the ER. The ER in typical fashion was no help, but I finally figured out on my own it was the spiro and the ambien I took at night. They were interacting somehow and once I stopped the ambien no more low blood pressure (too low I mean) and I do not know whay it was hanging over in the day and reacting with the spiro.

Some people, I think I may be one, and I had many patients say the same thing, that even though we aim for a low blood pressure for health reasons, when some get around 120/70 or so they would say they got dizzy and didn't feel good. Maybe some of us just need a little higher pressure to feel "normal". I saw alot of people say the same thing with blood sugars - when it was where we wanted it for the normal person, they said they felt horrible. A little high and they felt good. Go figure.

43 yo 5'10" 170lbs male PA-C and EMT-P w/ PA responded well to spiro and DASH (have gone from 25mg q day to 12.5 q day). PA led to development of a murmur, LVH, and combined with Hyperthyroidism a real CHF like lifestyle - which lifestyle was turned upside down in 2009, but now doing much better. Had an Average BP 160/120 (170/130 AVERAGE when TSH was also low!) for many years on 5 BP meds and now 130/80 on only 25mg/day spiro and DASH alone!

I suspect the DASH is having is effect and the spiro is too much. You have orthostatic hypotension. Are you taking your standing pressure? Be sure arm in on a support so center of cuff is at heart level. I prefer a Mayo stand but prob dont have one at home.Other option is to add a bit of salt back to diet. It is the running average of that am pressure that we want to focus on. Taken 3 x in am.Keep us posted. ok I'll take it 3x in the AM to see how it is...

I hope I don't need to add salt but since yesterday I am at 12.5mg of spiro and

today I had a BP of 110/70 ( checked twice )

At this pace I won't need spiro at all :S

However I have to say that I am DASHING extremly hard.

nothing I eat has other than natural salt.

as I have said I make my own bread, I eat everything LOW salt, and no processed

foods.

Dr Grimm.

What do you think is better to just take 12.5mg of spiro a day or add a bit more

salt ?

Today I have been at 12.5mg of spiro and my bp is still under control.

I would rather keep eating slatless and use less and less spiro.

But what's the medical recomendation ?

Checking sitting BP THEN standing after 1 min then stand up and down on ties for 10 times and recheck. I have been checked many times at both Dartmouth ED and White River VA ED for

changing B/P from laying down to sitting to standing. Never has been done with

and kind of supporting device.

Here are my values of today in the morning

I took the BP myself with an aneroid meter

and re-checked it with a digital wrist meter,

the error is of +-4mmhg.

LA: ( 3 takes aneroid 2 digital , average is presented here )

120/79

RA: (4 takes aneroid, 3 digital, average is presented here )

110/70

as stated before there's an error of +-4mmgh

This was taken right after waking up, sitting in bed.

there, seems to be a difference of 10mmhg between right and left.

HR around 70.

on 12.5mg Spiro 6.25am ( 11:30am ) 6.25pm ( 11:30pm ) the pressure was taken

today at 10am ( I sleep late due to my work )

Here are my values of today in the morning

I took the BP myself with an aneroid meter

and re-checked it with a digital wrist meter,

the error is of +-4mmhg.

LA: ( 3 takes aneroid 2 digital , average is presented here )

120/79

RA: (4 takes aneroid, 3 digital, average is presented here )

110/70

as stated before there's an error of +-4mmgh

This was taken right after waking up, sitting in bed.

there, seems to be a difference of 10mmhg between right and left.

HR around 70.

on 12.5mg Spiro 6.25am ( 11:30am ) 6.25pm ( 11:30pm ) the pressure was taken today at 10am ( I sleep late due to my work )

My definition of too low of a blood pressure is when you get dizzy. CE Grim MD. First, Congrats on the BMI going down.I am not on spiro and my BP is controlled under 120/80 without meds. I am not overweight. A few weeks ago I had a bp of 102/68 and pulse of 80. I felt so weird and thought I might be dehydrated. Dr. G thought I might need a bit more salt. I added in some more and felt much better and my pulse rate was back around 66. At this pace I won't need spiro at all :S

However I have to say that I am DASHING extremly hard.

nothing I eat has other than natural salt.

as I have said I make my own bread, I eat everything LOW salt, and no processed foods.

I have to say I am somewhat scared of giving the meds up, alltogether, I mean my

bp has been very low lately, I think I might add some salt but I really have no

idea what to do. :(

In the old days Dr. Kempner, Rice Diet founder, would cut salt till folks started to get low BP and then add back salt till went up again. They step down a bit till exact amt was found. And this wasin 1944 where there were no BP meds and he was treating folks with BPs of 250/150.See Ricediet.com for some of his publications.An earlier worker stated that when trying low salt diet unless you get to where your pts is complaining of low BP etc. you have not really restricted salt enough. Then add back.See no problem in stopping spiro and seeing. Yes I already checked it I do like the concept of the weightloss , or extreme

weightloss in 1 month :D

I doubt I'll go that low on the NA or Cals, but I'm hanging here, my bp seems to

be holding.

I have to keep dashing and checking to see if I will stop my meds or not :D

BTW I lowered the meds and I'm less dizzy :D !
