False-positive ARRs in normal women during the luteal phase only when DRC is used may explain the higher incidence of false-positive ARRs in hypertensive women than men and suggest the following:

1) plasma renin activity is preferable to DRC in determination of ARR and

2) new reference ranges for ARR that take into account gender and sex hormone levels are required.

Well I have only been saying this since DRCs came out. Good to hear an echo. Unfortunately no one has enough "normals" in say whites not to mention African Americans to have a normal value table.My and others research for about 40 years has shown that blacks have lower renin and higher aldos than whites but no lab that I have ever read ever discusses this part of the equation.Indeed if you are black and they use the Mayo numbers on you will have a large chance of being Dx ed with PA when you don't have it. Or if you use our AA numbers in Mayo you miss most PAs. Dr. Grim,

So does this mean if you are an African American female as far as

interpreting the results of your testing? Is this only for the screening

tests or also for the confirmation testing (suppression tests, etc?) I

would assume that an African American female would have an even higher

change of a false positive if tested at the wrong time of the month.

In females and African Americans, what do you suggest to confirm

diagnosis?

We have numbers from our lab but most labs don't use this assay. One should ask the lab. that is doing the assay. Our lab is no longer functioning. I would still use the aldo renin ratio as the start. Then ask the lab if they have any normal values in AAs. I would suspect other ethnic groups vary as well but never well tested.CE Grim MDDr. Grim,So does this mean if you are an African American female as far asinterpreting the results of your testing?
