Many times a doctor gets into practice and does things the way his or her mentor did it relating to the business of practice. Copying a mentor or a colleague, unfortunately, is often inefficient, antiquated or (innocently) illegal. Let's talk about that money that can get you feeling stuck in the middle between your doctor and your patients. Perhaps your doctor is struggling because he/she has set the practice fees too low; worried about the patients' ability to pay. Or maybe your fees are set high to maximize the third party reimbursement that may be available; but your doctor is cutting special deals, encouraging no out of pocket expense for patients, or setting up "time of service discounts" that far exceed the 5-15% discount the OIG recommends. All of these activities can cause problems for your doctor and your practice's success. We have the simple solution for you. In response, the doctor counsels, "Then don't do that!" In the real world, the doctor would likely have said, "Don't do that - and take these pills." The paradigm of healthcare has shifted dramatically from pain to performance. If you are practicing to the standard of care of the chiropractic profession, when a patient presents with a complaint, documenting pain is insufficient to establish medical necessity for your care. Objectively Measure and Document You must objectively measure and document the patient's level of function. What range of motion and muscle strength does the patient have and how does it compare with the peer-reviewed, published, normative data for those areas of function? Are there any areas of muscle imbalance that can be measured? How does any level of function below normal impact this patient's ability to perform activities of daily living? How will you go about monitoring these functions? And how does the patient feel about his condition? Is he anxious or depressed? Is he developing behaviors of pain avoidance? How will you measure this aspect of the patient's presentation? In order to establish unshakable medical necessity for the care that you will deliver, you must first objectively document the patient's level of function. Only then can you develop a treatment plan with a goal and a time frame for improvement that can be objectively documented upon re-examination. Medical necessity is based upon your ability to objectively document functional improvement. Across the country, chiropractors are losing sleep over concerns about "post payment audits." Post payment audits occur when an insurance company examines your patient records for objective documentation that supports the care you have delivered and been paid for! A lack of adequate documentation results in your having to return those payments. Don't get caught in the post payment trap. Document by measurement.
