Guest guest
ok to recapitulate. o, obese, with PA 25mg of spiro BID. Ok last week I got a NA in urine test 24 hours, it came with 190Mm/l with a volume of 1650ml. while dashing. However I think that because I cleaned the mug where I collected the urine with sodium hypoclorite it might have affected my values. so I re-took the test on a brand new mug, jar etc... here are the values of the new test: NA: 41Mm/l ( normal 40-220 ) K: 71.5Mm/l ( normal 71.5 ) If I'm not mistaken it means that I ate around 1035 mg of na that day, looking at my records of what I ate that day according to my calorie counter website I ate: ( caloriecount.about.com ) 724mg of Na during that day, and I would have to add 30mgs of sodium because of the water I drank: comes to a total of 754mg according to the website, and 1035 according to my urine, I could have left something I ate out, but it is somewhat a ballpark figure that can help me know if I'm on the right way of dashing. Now to DR grim what is a good NA/K relation, and exactly what does it means and what should we understand about it ? NA/K ratio here is 0.57. Thank you. Guest guest
No we need your urine volume to calculate the ENa.ENa = mMNa/L of urine x the volume of urine in liters. SAME FOR K.Using just the K/Na ratio you are doing very well as the goal is to at a minimum to have more K in urine that Na.The goal of DASH should be a ratio of about 1.6 K to 1 Na.you have 71.5/41 = 1.74 Good on Ya!.Still good to look at 24 hr excretion. ENa = 41 x urine volume in liters.Great work.CE Grim MDok to recapitulate.o, obese, with PA 25mg of spiro BID.Ok last week I got a NA in urine test 24 hours, it came with 190Mm/l with a volume of 1650ml. while dashing. Guest guest
Assuming you did a complete 24 hr urine and did not lose a drop. I assume you got the following instructions on how to do a 24 hr urine.1.When you start: pee and throw out. Record the time. Your bladder is now empty.2. Every thing you pee until 24 hrs are up should go into the jug.3. Record the exact time you end the urine. if not exactly 24 hrs the lab will need to adjust to 24 hr.So if all this was done then you are super DASHIng but that may be even better. Guest guest
Just wanted to thank Dr. Grim on his instructions on how to diagnosis PA in one day. I took his list to the GP and she just copied it on to a lab requisition. Question 1. Medication wash out period - I have been on Atenolol now for 10 days (except I took one on Day 3 as I was getting nervous about palpitations). Do you think I can go for the urinary on Day 14 or did I mess my self up taking the one Atenolol? I do not want to mess up this test. Question 2. Potassium intake prior to testing - First instruction in your "how to get a diagnosis" list says to eat a normal or high salt diet two weeks prior to urinary and serum tests for PA. What am I supposed to be doing about potassium intake. Keep it high or just normal? I have never had a serum potassium test. Guest guest
Just wanted to thank Dr. Grim on his instructions on how to diagnosis PA in one day. I took his list to the GP and she just copied it on to a lab requisition. Ideally you should be off all drugs for 2-6 weeks but if you need a med such as atenolol then that might overide the advice to stop all drugs. What I would suggest is that you go ahead as you are doing and do the test. It is easy to repeat a urine test.Question 1. Medication wash out period - I have been on Atenolol now for 10 days (except I took one on Day 3 as I was getting nervous about palpitations). Do you think I can go for the urinary on Day 14 or did I mess my self up taking the one Atenolol? I do not want to mess up this test. I am not sure if you have taken only one Atenolol or one a day?Question 2. I would suggest eating a normal K intake which should be at least 80 mM K a day. This has not been well studied either. Be sure your Dr. measures the Na, K and creatinine in the 24 hr urine.CE Grim MD Elaine "Just want to get my mojo back"To: hyperaldosteronism Cc: Clarence Grim Sent: Wed, January 19, 2011 10:02:41 PMSubject: Re: Re: My new lab tests now they make sense Assuming you did a complete 24 hr urine and did not lose a drop.
