Vern, Couldn't agree with you more regarding the PARQ conference being the point of intervention. It isn't done; culture within an allopathic office visit is so one sided that rarely is the doctor's suggestion for intervention questioned....much less asked 'if there is anything else I can do?" If that question is broached, a prescription for PT is extended, along with the pharmaceutical prescription, case closed....until the prescription needs to be refilled. They seem to be willing to suggest anything but structural intervention. One of the NPs I was acquainting myself with, when asked if I could serve the chirorpactic needs of her elderly commuity practice, looked at me and very innocently said: "I can't think of any of my patients who might need your care." One of the few times I admit to being speechless. I know they study the nervous system, how it does not equate continues beyond me. Of course I know it is all about money, but where does the sense of humanity come in? BJ's 'The Big Idea came across my desk this am; that piece can bring tears to my eyes. How can that concept be so resisted? Anyway: you are right on about focusing on the PARQ as the point of 'why isn't this included here?' avenue. If we are responsible for discussing any possible surgical intervention alternatives, don't they have the same legal burden? Particularly given the degree of the over-diagnosis that seems to be being practiced. (Secondary to malpractice fees?) Dear colleagues, We need a body on this work group relative to pharmacology as in the continued use of synthetic phyarmacologcial agents with their high level of adverse events is inappropriate when there are less invasive natural evidence based remedies. Not the least of which is spinal manipulation for acute, subacute, and chronic lower back pain which the state of Oregon now recommends. Trouble is they also recommend all the normal medications and medical doctors are simply unlikely to change there ways and will continue to view lower back pain episodes as simply a Vicodin deficiency! As per liability issues and defensive medicine understanding the significant side effects of the pain drugs vs spinal manipulation…..and the State of Oregon’s Evidence-based Guidelines for Low Back pain recommending spinal manipulation…the “defensive medicine” heat is going to be turned up on the medics who refuse to change there was as in never referring first for chiropractic care. As many of you know I work for NCMIC and OUM as per malpractice reviews/medical expert work……in my opinion Informed consent mandates that the physician have a PARQ conference with the patient to in fact achieve true informed consent. This means the doctor must fully describe the (P) procedure(s) he wishes to administer, provide any viable (A) alternatives, describe any material risks ® with the proposed procedure and the ask if the patient has any questions (Q). In my opinion failure for a medical physician to inform their patient with an episode of lower back pain that spinal manipulation is a viable alternative to the Vicodin or NSAIDs he/she prescribes constitutes failure to gain true in form. If in fact that patient goes on to have a serious adverse event to that medication (like death) constitutes negligence and the doctor’s butt is hanging out ripe for the plaintiff attorney’s kicking!! This is this issue I will bring up during my Evidence-based Guidelines Subcommittee(of the Health Evidence Review Commission) meeting later today. That the “coverage guidance” document we are drafting include a comment about spinal manipulation as a viable alternative to pharmacologic agents for lower back pain and a comment that provides a nexus “informed consent!” The “worm is about to turn!!” To The Continued Good Fight!
