23 yo male recently presented with:

* 1-month hx intermittent severe heart palpitations, occasional SOB, L scap "tightness", occasionally "feel fluid draining " L neck to L lat scapula, constant numbness L 4th-5th digits only when lie on left side. Recent visits to PCP/ER: EKG, Halter monitor wnl. Lab results pending. Pt told "it's all in your head". Family hx: 2 maternal grandparents died in their early 60's w/MI.

* severe anxiety related to recent job/housing/income loss ( moved back with parents), ongoing litigation, current health crisis, self-dx PTSD.

* severe postural distortion related to improper work ergonomics for past 3 years, mult segmental dysfunction C3-T8, L ribs 3-6.

Clear physical/mental overlay - will review lab results this week, continue chiro care 2 wks, possible ref.

I'd be interested to hear of your experience tx pts with similar s/s.

since immediate heart probs have been ruled out, sinus node aberrant firing - that would've shown up on halter monitor if he wore it for approx 2 weeks, EKG normal means even if he did have a heart episode, no heart muscle damage was done. He could still have episodic syncope related to intermittent vaso-vagal responses to stress. The positional symptoms might indicate a cervical involvement ( since presumably, the heart is ruled out - did he see a cardiologist?). Is there an MRI of the C-spine? Or x-ray? Was there any physical trauma to the C Th spine? Is his BP normal? Cholesterol normal? height weight normal?

You could instruct him in a home micro-current stim machine that he hooks up to the earlobes to change brain wave activitiy. it predominates alpha wave activity, more conducive to relaxation, lowering BP and reducing anxiety. I've given this unit to ex-military with PTSD (with good results)This treatment is in our scope (tho I caution you against using a PTSP diagnosis) and seems to be showing good response for episodic anxiety, situational depression and NMS symptoms. I have a company that I order the home units from. It allows me to sell them retail for approx $76 each. I can't recall the company name or wholesale price, but you can call my office to ask the secretaries about it. It's called a micro-mode. I would caution him about using it over the heart area because of his symptom profile. Have him use it in your office with you present the first couple times. bill assisted stim. Description of use in chart should be for symptoms associated with vaso-vagal pain, tingling in UE, cervical and thoracic pain. By clearing those symptoms, the anxiety associated with the symptoms, may help him overall.

The price is so reasonable, I usually tell patients they can check the unit out and if they purchase it, I apply the weekly rental fee to the purchase price. But they have to buy the same unit they rented. And they have to pay for he rental fee as they go. I charge $25/week. This unit can also be used on NMS symptoms. It's excellent. I've only had 2 patients return it in the last 4 yrs. Others have opted to keep the unit.
