1. I have not seen good data on how long one must be off eplerenone before RAAS returns to baseline in PA.Please give us her complete story with numbers. You cannot give us too many details.How many times a night does she get up to pee for example?Testing is no different. I like to do an over night dex suppression of plasma aldosterone which goes to zero in GRA.If she has GRA then all first degree relatives should also be tested and there are genetic tests for this available from Yale.We can set up a consulting arrangement so I can work with your Drs and you in guiding the work up and long term management. My 13 year old is currently in the hospital and being worked up by multiple specialists, including endocrinology. He is testing her for PA and she has been off Eplerenone for 14 days. He did a renin/aldo draw a couple of nights ago to test for it. I gave him the information that you shared regarding best practice for the testing and he said he may do it again based on that information, however he said that the Eplerenone had cleared by now. Could you share with me why the time line of 4 or more weeks before testing is used so I can pass it along? I want to make sure that we rule PA in or out based on correct testing.Also, whatever my daughter has could be genetic, so would the testing for hyperaldosteronism be any different for patients with a genetic cause?Thanks so much,Cindi
