(here's the form http://www.ssa.gov/online/ssa-561.pdf ) With this you will also need to request a further examination of your medical records, doctor statement, your work record, education, age, and in the long run provide proof you are UNABLE TO WORK. Those last 3 words are the KEY. You might want to look into obtaining legal representation now. You can find attorneys in your area by contacting http://www.nosscr.org/refer.html It seems to me that the SSA did not fully evaluate your application nor did they attempt to contact you or obtain additional medical records. As for directing you to your Congressman - that is just a place to contact to tell about your 'problems' dealing with SSA!
