Guest guest Posted January 2, 2006 Report Share Posted January 2, 2006 , From your doctor/s get: 1) Letters saying why you CANNOT work any longer. (make sure the doctor does not use the word " disability " but instead writes about all the tasks you cannot do that are connected with your work) 2) Get WRITTEN copies of every test, procedure you have had - MRI, Xrays, DNA test, EMG/NCV, nerve biopsy, etc. 3) Once you have these, apply for disability using the on-line application. At the end of it there is a cover page which you print out and attach it to the copies of written reports from your doctor - remember, everything MUST be about why you CANNOT work - not using the word " disability " . In describing your CMT in the application, state it is a PERIPHERAL Neuropathy. Also in that application, state if you use AFOs, other devices, have breathing problems, or other medical issues from CMT/PN. If you have been receiving PT or OT mention that too. 4) After you file your application on-line, print out a copy of it for yourself. You will also need a complete list of your work history for the last 10-15 years, describing the work you did, giving the employer's name, address, etc. 5) There is also a section on what doctors you have seen and if you have been hospitalized, list the hospitals.
