I really think I'd just leave it off and wait for the casting. I don't think it will be so bad waiting a month, but you have to trust your own gut. They'll do amazing things on the AMIL table with the cast - it's taken a few to get Jack's ribs looking symmetrical again, but they really do in this cast!! The window is essential in flattening the ribhump while allowing the ribs on the window side to return to a more normal state. If the cutouts in the front of the brace are not done properly, I would be concerned about rib cage deformity. I don't know what to tell you...definitely get Dr. K's opinion. Maybe you can even email him some pictures. Be sure to let us know what he says. Finn received his brace three weeks ago and has been wearing it for at least 19 hours a day since he got it. The brace was molded using a Risser table and they tried to get as much correction in it as they could. He has the rib hump in the back, but I have never been told what his rib rotation was. I don't think they even measured it (Shriners Tampa). Now that he has been in the brace for about three weeks, it seems his hump in the back is now pushing out the front. The back is flatter. Is this what is suppose to happen? It doesn't look right to me. We are going for casting with Dr Khoury on Feb 28th but a lot could happen in that time. I called Shriners and (Dr Khoury's nurse)but have not heard a response. Finn has had a recent growth spurt also so I don't want to take him out of the brace if he will curve more. Finn doesn't hold his head up in the brace either and reverts back to his severe Torticollis position. Out of brace he is doing pretty well holding his head normally. We see a chiropractor and PT a couple of times a week so I think we can hold on till February without the brace, especially since it seems to be a set back. Sounds like your going with gut on this one, and thats probably the best thing to do. You can access the situation best, since you're the mum and can see that the brace might not be doing a good job. I remeber when my daughter Olivia was around 4/5 and wearing a brace...The brace did ~not~ have the right windows and I could see that her ribs were beginning to flare out of the tummy hole! I had to insist that the orthotist in our area added a leather covering over the tummy hole, to stop the ribs from flaring out of the hole. I can still see her flared ribs (5 yrs later) and I know that this is what caused it. Dont mean to alarm you or anyone on the group. Just sharing our experience. I talked with the orthotist today that made his brace and he said to come back in and he would " blow out " the areas of concern. I also talked with from Dr Khoury's office and she said to take it back to the brace shop. She said she would like him to stay in the brace if possible and that I should not be to worried because the brace would not deform his ribs between now and February 27th when I see them. I am going to watch it very closely. I get very frustrated because there are so many conflicting opinions. I will be very happy when we go to our Clinic appointment and from then on, I can rely on Dr Khoury, who is very experienced in IS. Guest guest Posted January 25, 2008 Report Share Posted January 25, 2008 Hi , We actually just noticed the head tilt with the brace on not too long ago. We haven't asked her doc about it yet. Kate has been wearing a brace since mid-November, however we lost some time because her Ortho was not happy with the first one we got so we had to have another one made at a different place. I worry about whether it's working, we haven't had any xrays since she first got it. Kate is also 20 months old and we are beginning casting next month at Shriners SC. Keep me posted about how Finn does with the head tilt in the cast, I'll do the same. Best of luck! She has full rotation as well and a slight tilt when the brace is off, but as I mentioned it's much more noticible when the brace is on.
