If the link that I supplied, in my previous posting does not work, use the

following link.

On the page that the link takes you to, click on the line: " Download

Monograph " , and it will start the download of the CME PDF file.

If, even THIS link does not function, just email ME, directly, and I will

email you the CME PDF file that I saved on my computer. Doctors never make it

easy for you to know what they know.
