Or has he been set at 20 degrees now for a few casts in a row? I think a lot of docs are willing to stop at anything under 20 degrees, because in real scoli. terms, anything under 20 degrees is not really considered scoliosis. Sure, 27 months is older, but not too old to get some correction more correction. How is the rotation? The rotation should be pretty much gone to stop casting. I would definitely contact Dr. . How fun for you and your boys to eat ice-cream and popsicles without worry about a messy cast. Did you get some pics.? Surprisingly, when that last cast came off he had achieved more correction. Dr. S didn't give me an exact number, but he said much less than 10 degrees and essentially straight. Dr. K had told me normally he didn't go to a brace until 5 degrees or less because he had had two children who were 10 degrees get worse when they went to a brace. Ian was going to be an exception only because he had remained at a constant 10 degrees for so long. As it is, I am nervous that he isn't doing well in his brace, but I am sure we all probably get nervous making the transition. If you are still getting correction, I would ask to continue getting casted. I dunno...tough call. Just knowing that some kids got worse when they moved to a brace at 10 degrees makes me nervous...even with Ian who was less than 10. I guess I lean towards being more conservative. She wants me to send last week's Xray for Dr. to see. Now I just wait on ish Rite to get it to me. My concern is they are S L O W. Your comment about stopping at 20 because in real scoliosis terms-well that is how his doc thinks. I don't agree. had good improvement with the first 3 casts and then just basically stay the same. So that is why his doc feels the way he does. I am just scared to take him out of cast at this point. Rotation? I truly do not know. We did notice his rib hump is almost gone. I barely noticed it. Still seems high? Maybe as you said there hasn't been much improvement in the past few casts and the doc feels that's the best that can be achieved. With Max, Dr. told us our goal is to just get Max to 40 degrees out of cast (currently 60). Then Max will go into a brace. If the curve progresses again in the brace they would put him back in cast. If he can hold at 40 degrees out of cast he would not need growing rod surgery.I know and Max are 2 different children with 2 different curves ('s more classic, Max more unusual). But we do see Miss Mehta tomorrow morning and I pan to ask what her thoughts are on the degree of correction (if any) she feels Max can achieve. And I'll ask to expand on why that degree and I'll let you know. Dr. Sturm did tell me at one point when I asked about how we determine when/if Max will need surgery. He said there is no magic number. He said it is determine at the progression rate of the curve. If it progresses highly very rapidly, then surgery will happen sooner than later. If casting can slow it down, then surgery may be delayed or not happen at all. Perhaps this concept is similar to going from casting to bracing. Good Luck and keep us posted. You'll continue to be in our thoguhts and prayers. PS - I'm so nervous to give Max an ice cream cone. I've never given him a popsicle. I just couldn't remember what exactly it was. I wouldn't say we are still getting correction with this style of cast. It is just holding it. A question I have for Dr. is does he think at this point he would able to get more correction than what his current doctor has achieved. We'll see what he thinks. I too would rather be conservative and recast than go ahead with the brace. I just couldn't remember what exactly it was. I wouldn't say we are still getting correction with this style of cast. It is just holding it. A question I have for Dr. is does he think at this point he would able to get more correction than what his current doctor has achieved. We'll see what he thinks. I too would rather be conservative and recast than go ahead with the brace. I do know there will be a point that the cast will only do so much. I just don't think we should stop yet. And it may just be time to finally change doctors. Please let me know how things go tomorrow. I know you are anxious to hear what Ms. Mehta thinks. You should let Max have ice cream, Popsicles and all the messy stuff tomorrow. You guys have fun without the cast and get lots of cast free hugs. I look forward to hearing about the whole ETP and what she thinks about Max's treatment. I will be praying. And I'll ask to expand on why that degree and I'll let you know. Dr. Sturm did tell me at one point when I asked about how we determine when/if Max will need surgery. He said there is no magic number. He said it is determine at the progression rate of the curve. If it progresses highly very rapidly, then surgery will happen sooner than later. If casting can slow it down, then surgery may be delayed or not happen at all. Perhaps this concept is similar to going from casting to bracing. Good Luck and keep us posted. I am not sure about a certain age when it doesn't work anymore, but I am sure there is a point when it is just harder to get correction. Every child is different and I think that makes it really hard. Maybe someone will chime in and inform us on the age thing. I am ready for to be in brace, but at a much lower degree. Especially after seeing the out of cast Xray from Wednesday. Of course it looks better from when he first started,but it was measured at 20 degrees. Is it just me or is it too soon to start bracing? Shouldn't it be a lower number? The past 3 casts has stayed the same. No improvement. Just basically held his curve. His doctor feels at his age, 27 months, he has improved as much as he will and the cast will not do any more. He stated that if his spine got worse in the brace we may have to cast for a while, brace for while and go back and forth. Once he starts wearing the brace he would not be seen for 6 months. Is that a really long time not to check on his spine? Back in March Dr. looked at all of 's films and felt like we were getting good results. Should I again have him look at the most recent films and give me his opinion? Would he do that again? Is it too late for the cast to continue to give correction? Some of you know my highs and lows with his doc. I felt much better after Dr. looked everything over, but now starting a brace I am very scared. I don't want to lose what we have achieved by putting him in a brace too soon. Has anyone had experience with bracing too soon? ANY thoughts would be very helpful. Guest guest Posted August 8, 2007 Report Share Posted August 8, 2007 hi! as jennifer mentioned (hi jennifer!), my son lucas did undergo casting with dr. sanders from the time he was 2 1/2 yrs old to 4 1/2 yrs old. my son's degree was much worse than your daughter's. he was in a total of 7 casts each lasting between 3-4 months between changes. it is my understanding that it is best to cast (to get correction) under 2 when children are growing at their fastest, but up to 5 is also done. over 5, i believe that it is more the maintain and avoid/delay surgery than to correct since growth is much reduced. if you would like to see some of lucas' pictures, they are under the photos section in the folder lucas. we decided against going into a normal tlso brace from erie at the end of the casting process last summer. we still keep in contact with dr. sanders and the folks at erie. it was like our second home for 2 years. lucas still thinks it was a vacation even with the anesthesia and casting! he just loved the playroom. i know i'm not as active on this board, but feel free to ask questions, and i would be happy to answer them as best as i can. deshea ---- Original message ---- >Date: Wed, 8 Aug 2007 09:49:57 -0400 > >Subject: Re: 6th cast/brace? They didn't have the casting technique back then, but it sounds like you have a fairly good plan. The purpose of the brace is only to stop the curve from progressing while grows. Most Dr's will suggest that the kids wear a brace at 20 degrees. If his little back were a lower number (under 20 deg) they would use the " wait and see " approach, to see if his back gets worse and then start to brace at 20 degrees. Every 6 mos. is " standard " when wearing a brace. I felt the same thing! It was an Emergency to get my son seen, then they put him in a brace and said... " we'll see you in 6 mos!! " Crazy, isn't it. Just monitor his little back. We did some nuero-muscular therapy (massage therapy). Swimming has been awesome for my son's back!!! (We've had the most improvement after the summer swim team). The beauty of the back brace is...you can take it off and get your hugs... it's amazing how they adapt. It is harder for the moms. I wonder if he thinks this is all the better it can get? Regardless...in the last 8 years my son's curve (without casting and with wearing a TLSO brace and other non-traditional things) his curve has been as high at 59 and 53 degrees and as low as 19 and 15 degress. He was 2 when diagnosed and is now 10 years old and 5 feet 5 inches tall, so there has been a bunch of growth. If there wasn't any improvement maybe take a short break...have him swim, swim and swim some more...lessons, etc. Then maybe try and cast him again to see if you can get more correction. Much to my surprise they also made a mold for a brace that he will start wearing September 11. So he will be in this cast for 6 weeks. ???? I knew there was a chance, but didn't think it was time. Especially after seeing the out of cast Xray from Wednesday. Of course it looks better from when he first started,but it was measured at 20 degrees. Is it just me or is it too soon to start bracing? Shouldn't it be a lower number? The past 3 casts has stayed the same. No improvement. Just basically held his curve. His doctor feels at his age, 27 months, he has improved as much as he will and the cast will not do any more. He stated that if his spine got worse in the brace we may have to cast for a while, brace for while and go back and forth. Once he starts wearing the brace he would not be seen for 6 months. Is that a really long time not to check on his spine? Back in March Dr. looked at all of 's films and felt like we were getting good results. Should I again have him look at the most recent films and give me his opinion? Would he do that again? Is it too late for the cast to continue to give correction? Some of you know my highs and lows with his doc. I felt much better after Dr. looked everything over, but now starting a brace I am very scared. I don't want to lose what we have achieved by putting him in a brace too soon. Has anyone had experience with bracing too soon? ANY thoughts would be very helpful. I have heard that swimming helps tremendously for the spine. I can't wait for to wear a brace and be able to enjoy water. At what degree is your son's curve these days? Casting is all we know at this point. I still feel could achieve more correction if casting was continued. We had success with his first 2 casts and then basically nothing. Right now I feel it is time to move on.............as in change doctors. Ms. Mehta's techniques and the AMIL frame have proven to help young children. deserves to be one of those children. Our doctor now is not Mehta trained. Again thanks for sharing. Regardless...in the last 8 years my son's curve (without casting and with wearing a TLSO brace and other non-traditional things) his curve has been as high at 59 and 53 degrees and as low as 19 and 15 degress. He was 2 when diagnosed and is now 10 years old and 5 feet 5 inches tall, so there has been a bunch of growth. If there wasn't any improvement maybe take a short break...have him swim, swim and swim some more...lessons, etc. Then maybe try and cast him again to see if you can get more correction. Playing in the water, held his 1st ice cream cones and Popsicles!! He and his brother had a blast. And then they enjoyed a REAL bubble bath. And I loved the cast free hugs. His little back " looked " great. His rib hump is almost gone. A normal person would not have even notice it. Thursday we went back to have his 6th cast applied.
