One dilemma lies with the large number of MSM who are poz and don't know it. In a major US metro area that can be 30 - 50% of guys who are poz. So, there is the parallel responsibility of all men who have sex with other men to get tested regularly and to know their status and to get treated. The positivity rates are 50% and more in the African-American populations of Chicago and DC, according to some surveys. That in part reflects the large number of guys on the DL. Treatment messages often don't get to them, and they are much less likely to go to be tested. This is a socio-cultural and socio-economic reality, unfortunately.

And then there are those who lie, in order to get sex.

I think that one way to end the epidemic of infected individuals is to get as many people tested as possible, using financial and other incentives when necessary, and then to TREAT EVERYONE with a high viral load, independently of CD4 count.
