I thought it would be a good idea for people with younger children to post suggestions as well. I'm reposting Conner's IEP modifications from 6th grade, his biggest issue at the time was fatigue. I'd love to hear some of the modifications used for younger and older children. Conner missed all of 7th grade due to his bone marrow transplant. The school provided him with a laptop, a printer, a set of text books/work books, literature books on tape and a tape player. He was not required to turn in any work or tests, he and I worked at our own pace. He was also exempt from state mandated testing. Due to his previous grades they allowed him to skip to 8th grade after BMT. Conner still has an IEP in place (11th grade), but he doesn't need much support at this time. Conner - 6th Grade - IEP Modifications Instructional Modifications and General Information: a.. Conner will not be penalized for absences or lose credits towards future graduation. b.. Homework assignments will be adjusted for extended absences or if suffering from fatigue. c.. Conner will not take a test or quiz on the day following his return from an absence or illness. d.. Conner will be allowed extended time on tests. e.. Poor handwriting will not be penalized and keyboard training will be provided, due to problems with fatigue. A color-coded keyboard is preferred. f.. Study guides will be provided for test preparation. g.. Conner will receive daily assistance with organization and assignment requirements. h.. Preferential classroom seating will be utilized. i.. A complete set of textbooks will be provided for home use. j.. Conner will be allowed additional time between classes, as needed. k.. Conner will be allowed unrestricted restroom privileges. l.. He will be allowed to rest, as needed. m.. He will be allowed to carry his own water bottle. n.. He will be allowed to change seats during class to avoid sitting near someone who seems ill. o.. Conner's status will be monitored on school trips and provided with periods of rest, as needed. p.. Conner should participate in physical education, as tolerated. However,he should avoid contact sports or activities that would increase a hit to the chest, unless appropriate padding can be placed over his port-a-cath. (we ultimately decided to exempt him from PE) q.. Conner can not participate in any physical activity during times when his port-a-cath is accessed for IV therapy. r.. Conner will utilize the resource room study hall for assistance with math curriculum. s.. Conner will be allowed to use a calculator and product finder to check work and for tests. t.. Conner will not share objects with other students, such as art supplies, music instruments or eating utensils, unless they can be properly cleaned and disinfected. u.. Conner will have minimal exposure to animals and no physical contact with reptiles or birds; he will not be expected to clean up after them. v.. Conner reserves the right to avoid doing tasks that would put him at high risk of being exposed to germs. w.. Staff will make an effort to not call attention to Conner's illness in the presence of other students. x.. Opportunities will be provided for Conner to spend time with the school counselor. y.. If Conner is ill or injured, he should see the nurse immediately and parents should be contacted right away. z.. If the nurse is available, she should go to Conner when he needs assistance, rather than exposing him to someone with an illness in the nurse's station. aa.. A fever of 100.5° F or greater is very serious for Conner, it should be reported to parents immediately. However, Conner's ability to mount a fever response is impaired and a lack of fever does not diminish the severity of an illness. ab.. The school will call 911 if Conner becomes seriously ill or unstable, such as a lack of consciousness, altered mental state, seizure, difficulty with motor function, etc. ac.. The school will notify parents of communicable diseases within the school so that Conner's physicians can determine if he needs to be placed on homebound status. The most serious risks being chickenpox, strep throat, influenza, measles, meningitis, hepatitis, etc. ad.. Parent/teacher conference will be held within the first three weeks of school. ae.. Monthly progress reports will be provided to parents. af.. All of Conner's teachers will be notified of his condition prior to the first day of school. Hope this helps!
